Clinical Study Of Xiaoxianxiong Decoction With Modified Recipe In The Treatment Of Phlegm-Heat-Stasis-Type Coronary Heart Disease With MS Under The Guidance Of Target Drugs

Objective: To explore the clinical efficacy of Xiaoxianxiong Decoction in the treatment of phlegm-heat stasis-type coronary artery disease complicated with metabolic syndrome and changes in related lipid metabolism indexes under the guidance of the "target drug of returning to the meridian",aiming to improve the clinical efficacy of the treatment of CAD and find new ones prevention strategy.Methods: A total of 66 patients with CAD of phlegm-heat stasis type combined with MS were enrolled in this study.They were grouped according to the random number table method and divided into observation group and control group,with 33 cases in each group.The control group was treated with basic western medicine combined with Xiaoxianxiong Decoction,and the observation group was treated with traditional Chinese medicine(hawthorn,red yeast rice,lotus leaf)on the basis of the control group for 12 weeks.After treatment,the patient’s serum NO(nitric oxide),blood lipids,waist circumference,total score of TCM syndromes,electrocardiogram curative effect,angina curative effect and nitroglycerin arrest were comprehensively analyzed,and adverse reactions were observed.Use SPSS25.0 statistical software for statistical analysis and draw conclusions.Results:1 Evaluation of the efficacy of angina pectoris: The severity of angina pectoris in the two groups of patients before treatment was tested,and there was no statistical difference(P>0.05),which was comparable.After treatment,the observation group was better than the control group in improving AP curative effect,and the difference in AP curative effect was statistically significant(P﹤0.05).2 Evaluation of TCM syndrome scores: The severity of TCM syndromes of the two groups of patients before treatment was tested,and there was no statistical difference(P>0.05),which was comparable.After treatment,the two treatments can improve TCM symptoms and reduce TCM syndrome scores,and the observation group is better than the control group,and the difference in the efficacy of TCM syndromes is statistically significant(P﹤0.05).3 Evaluation of nitroglycerin reduction: After treatment,the two groups of treatments can reduce the amount of nitroglycerin,and the observation group is better than the control group,the difference is statistically significant(P﹤0.05).4 Evaluation of ECG curative effect: The total effective rates of the control group and observation group were 56.67% and 60.00%,respectively,and there was no statistical difference(P>0.05).The two groups had the same level of ECG curative effect.5 Observation index changes:(1)Waist circumference change:Before treatment,there was no statistical difference in waist circumference between the two groups(P>0.05),which is comparable;after treatment,both groups can reduce the waist circumference of patients,and the observation group is better than the control group,The difference is statistically significant(P<0.05).(2)Blood lipid changes: Before treatment,the four levels of blood lipids(TG、TG、LDL-C、HDL-C)of the two groups of patients conformed to the normal distribution(P>0.05)and were comparable;after treatment,the four levels of blood lipids of the two groups of patients improved,including TG,TG,the level of LDL-C decreased and the level of HDL-C increased,and the observation group was better than the control group,the difference was statistically significant(P<0.05).(3)Change of serum NO concentration: Before treatment,the difference in serum NO content between the two groups of patients was not statistically significant(P>0.05),which was comparable;after treatment,the serum NO content of both groups increased,and the observation group was better than the control group.The difference was statistically significant(P<0.05).6 In terms of safety: the subjects had no significant changes in vital signs(body temperature,respiration,pulse,blood pressure,heart rate),blood and urine routine,liver and kidney functions during the 12-week experiment;no drugs appeared Adverse events such as allergic reactions and gastrointestinal discomforts indicate that Xiaoxianxiong Decoction combined with conventional Western medicine is safe.Conclusion(s): 1 The modified Xiaoxianxiong Decoction based on the guidance of "guijing target medicine" can significantly improve AP symptoms of CAD&MS patients with phlegm-heat stasis type,reduce TCM syndrome scores,improve TCM syndromes,further regulate blood lipid levels,increase NO content,and security is good.2 In this study,Xiaoxianxiong Decoction was selected as the basic formula,which was compatible with phlegm and turbidity(Gualou,Scallion White,Pinellia),clearing internal heat(Coptis),and removing blood stasis(Danshen,Panax notoginseng powder).The medicine idea is to choose hawthorn,red yeast rice,and lotus leaf that have good lipid-lowering and weight-reducing effects and belong to the spleen and stomach meridians.The results prove that the drugs selected under the guidance of the "guidance target drugs" can target the regulation of CAD&MS patients.The blood lipid level is effective for CAD patients with abnormal lipid metabolism and obesity,and it is worthy of clinical promotion.
