Based On Bibliometrics And Evidence-based Medicine To Explore The Clinical Effect Of Peitushengjin Therapy On Chronic Obstructive Pulmonary Disease In Stable Stage

Objective: 1.Based on the method of bibliometrics,this paper analyzes the current situation,hot spots and future development trend of clinical research on the treatment of COPD in stable phase by peitushengjin therapy,in order to provide some reference and research direction for clinical and scientific research.2.Based on the method of network Meta-analysis,the efficacy of related indicators of Peitu Shengjin prescriptions with frequency>10 in bibliometrics in the treatment of stable COPD was evaluated,and the advantages of corresponding intervention measures were compared to provide appropriate medication reference and evidence-based medicine basis for the treatment of stable COPD with traditional Chinese medicine.Methods: 1.Bibliometrics part: using computer retrieval China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP Database(VIP),Chinese Biomedical Literature Database(CBM)in the treatment of stable COPD by reinforcing earth to generate Metal method of literature,literature screening according to the established inclusion and exclusion criteria,using Cite Space 5.7.R5 software to finally included in the literature of the authors,institutions,keywords,such as co-occurrence analysis,clustering,burst,time clustering analysis of keywords,draw the relevant network map,realize the visual analysis of the included literature.2.Network Meta-analysis: CNKI,Wanfang Data,VIP,CBM,Pub Med,Embase,the Cochrane Library and Web of Science databases were searched by computer.Randomized controlled trials(RCTs)of Peitu Shengjin Decoction in the treatment of stable COPD with frequency>10 in bibliometrics were retrieved.Note Express3.5 software and Excel software were used for literature screening and data extraction.The bias risk assessment tool of Cochrane evaluation manual 5.1.0 was used to evaluate the quality.Rev Man5.4 and Stata17.0 software were used to perform traditional Meta-analysis and network Meta-analysis on each outcome index.Results: 1.Bibliometrics: A total of 530 clinical research literatures were included.The results of visual analysis showed that the number of publications in this research field showed a fluctuating upward trend,reaching the peak in 2016,and the top 10 journals were mainly national journals.Through the co-occurrence study of the author ’s cooperation network,it is found that a research team with “Lin Lin ” “Liu Xiaohong” “Li Fengsen” as the core authors has been formed.The internal links of the team are close,but the links between the teams are poor.The research institutions are mostly traditional Chinese medicine universities and affiliated hospitals.The keywords were analyzed by co-occurrence,clustering and burst analysis,and the external treatment of traditional Chinese medicine with frequency > 5 was obtained: acupoint application,acupuncture and acupoint injection;observation indicators with frequency> 5: lung function,quality of life,CAT score(chronic obstructive pulmonary disease self-assessment test),6-minute walking test;prescriptions with frequency> 10: Liujunzi Decoction,Shenling Baizhu Powder,Bufei Decoction,Buzhong Yiqi Decoction.The research hotspots mainly include ancient prescription addition and subtraction,self-made prescription,acupoint application,acupuncture,lung function and so on.2.Network Meta-analysis: Finally,45 RCT literatures were included,including 3670 subjects.The experimental group involved 4 interventions(Liujunzi Decoction combined with conventional western medicine group,Shenling Baizhu Powder combined with conventional western medicine group,Bufei Decoction combined with conventional western medicine group,Buzhong Yiqi Decoction combined with conventional western medicine group),and the control group was treated with conventional western medicine.2.1 The results of traditional Meta-analysis showed that:(1)In terms of clinical total effective rate,Liujunzi Decoction,Shenling Baizhu Powder,Bufei Decoction,Buzhong Yiqi Decoction combined with conventional western medicine were superior to conventional western medicine group in the treatment of stable COPD;(2)In terms of improving FEV1,Liujunzi Decoction,Shenling Baizhu Powder,Bufei Decoction combined with conventional western medicine in the treatment of stable COPD is better than conventional western medicine group;(3)In terms of improving FEV1 %,Liujunzi Decoction,Shenling Baizhu Powder,Bufei Decoction,Buzhong Yiqi Decoction combined with conventional western medicine in the treatment of stable COPD were better than conventional western medicine group;(4)In terms of improving FVC,Shenling Baizhu Powder and Bufei Decoction combined with conventional western medicine were superior to conventional western medicine group in the treatment of stable COPD;(5)In improving FEV1/FVC,6MWD and CAT scores,Liujunzi Decoction,Shenling Baizhu Powder,Buzhong Yiqi Decoction combined with conventional western medicine were superior to conventional western medicine group in the treatment of stable COPD.2.2 The results of network Meta-analysis showed that: the method of strengthening earth to generate gold combined with conventional western medicine in the treatment of stable COPD in improving the total effective rate of clinical efficacy was: Buzhong Yiqi Decoction > Shenling Baizhu Powder>Bufei Decoction> Liujunzi Decoction;the order of improving FEV1 was:Bufei Decoction> Shenling Baizhu Powder> Liujunzi Decoction;the order of improving FEV1% was: Buzhong Yiqi Decoction> Shenling Baizhu Powder>Liujunzi Decoction> Bufei Decoction;the order of improving FVC was:Buzhong Yiqi Decoction>Bufei Decoction> Shenling Baizhu Powder>Liujunzi Decoction;the order of improving FEV1/FVC was: Bufei Decoction> Buzhong Yiqi Decoction> Shenling Baizhu Powder> Liujunzi Decoction;the order of improving 6MWD was: Buzhong Yiqi Decoction>Liujunzi Decoction> Shenling Baizhu Powder;the order of improving CAT score was: Liujunzi Decoction>Buzhong Yiqi Decoction> Shenling Baizhu Powder.Conclusions: 1.Bibliometrics: The overall number of papers in this research field shows a fluctuating upward trend.The published journals are mainly national journals,and several research teams have been formed.The research institutions are mostly universities of traditional Chinese medicine and their affiliated hospitals.The research hotspots mainly involve internal and external treatment of traditional Chinese medicine,clinical efficacy analysis and quality of life of patients with stable COPD.2.The network Meta-analysis part: Buzhong Yiqi Decoction combined with conventional western medicine has more advantages in improving the total clinical effective rate,FEV1%,FVC and 6MWD in the treatment of stable COPD;in terms of improving FEV1 and FEV1/FVC,Bufei Decoction combined with conventional western medicine has more advantages in the treatment of stable COPD;in terms of improving CAT score,Liujunzi Decoction combined with conventional western medicine has more advantages in the treatment of stable COPD.In clinical application,it is necessary to choose the superior treatment plan according to the general condition of patients.
