| Objective:A questionnaire survey was conducted on patients undergoing PCI for the first time to understand their needs for TCM continuous nursing after discharge.Based on the needs for TCM continuous nursing of patients,combined with literature research and guided by the concept of TCM "treating no disease",the Delphi method was adopted to construct TCM continuous nursing plan for patients after PCI.Methods:1.A cross-sectional survey was adopted in this study.138 patients who underwent PCI for the first time in the first ward of Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine were selected as the research objects,and a questionnaire was prepared by ourselves to understand the needs for TCM continuous care of patients after PCI.2.Conduct literature research,through systematic retrieval of domestic and foreign guidelines related to cardiac rehabilitation and TCM nursing of patients after PCI,expert consensus,systematic evaluation and randomized controlled trials,strictly evaluate the quality of literature,extract and summarize evidence,and provide a basis for the construction of TCM continuous nursing programs for patients after PCI.3.Based on the results of literature research and combined with the results of the previous investigation of patients’ needs for TCM extended care,the TCM extended care plan for patients with coronary heart disease after PCI was initially formed.17 cardiovascular experts from 4 provinces were selected to optimize the plan by Delphi method,and a total of 2 rounds of expert letter consultation were conducted.According to the views and suggestions of experts,this program was modified repeatedly,and finally formed the traditional Chinese medicine nursing program for patients with coronary heart disease after PCI with basically the same opinions of experts.Results:1.After PCI,patients had a high degree of awareness and acceptance of TCM continuous care.84.0% of patients believed that TCM nursing was scientific,and 86.23% of patients were willing to accept TCM continuous care.The highest recognition degree of TCM nursing content is the nursing technology with TCM characteristics,the recognition degree is 72.46%,the highest recognition degree of TCM nursing technology is moxibustion,the recognition degree is 60.87%.2.Through rigorous literature search and quality evaluation,43 literatures were finally included,including 6 guidelines,9 expert consensus,1systematic review and 27 RCTS.After extracting and summarizing the evidence,the draft plan of 6 first-level indicators,16 second-level indicators and 39 third-level indicators was preliminarily formed,including daily life conditioning(2 indicators),medication conditioning(8 indicators),emotional conditioning(6 indicators),traditional Chinese medicine diet conditioning(5indicators),traditional exercise(8 indicators),traditional Chinese medicine nursing technology(10 indicators).3.Through two rounds of Delphi expert letter consultation,the questionnaire recovery rate of the first round and the second round was 100%;The opinion raising rates were 82.35% and 11.76%,respectively.The expert authority coefficients(Cr)were 0.918 and 0.911,respectively.Kendall coordination coefficients W were 0.336 and 0.370,respectively.Coefficient of variation(CV)of all subjects was < 0.25.The final plan consists of 5first-level indicators,which are disease monitoring and first-aid management,drug administration based on syndrome differentiation,diet administration based on syndrome differentiation,operation based on syndrome differentiation and nourishing based on syndrome differentiation.There are13 second-level indicators and 49 third-level indicators.Conclusions:1.Patients with coronary heart disease after PCI have a high degree of awareness of traditional Chinese medicine nursing,and their acceptance of traditional Chinese medicine continuous nursing is ideal.Among them,nursing techniques with traditional Chinese medicine characteristics are more popular among patients.2.This topic is based on literature research and combined with patient needs,through the scientific method of Delphi expert letter consultation to build a continuous care program of traditional Chinese medicine for patients with coronary heart disease after PCI.The experts are authoritative and positive,and the constructed continuous care program is comprehensive and scientific and rational,which can provide reference for carrying out continuous care of traditional Chinese medicine for patients after PCI in the future. |
