The first department:Tracing the origin of the theory of treating liver depression and spleen deficiency IBS-D from the theory of liver and spleen Objective:Through combing the ancient Chinese medicine literature,clarifying the origin and development of diarrhea and the basic theory of liver and spleen,providing in-depth ideas and methods for clinical treatment of diarrhea of liver depression and spleen deficiency.Methods:Using the method of literature research,we have extensively collected the academic viewpoints and literature data of various doctors on the treatment of diarrhea and the treatment from the liver and spleen in traditional Chinese medicine,conducted an in-depth discussion on the source of diarrhea,and systematically discussed liver depression and spleen deficiency The etiology,pathogenesis and dialectical points of type diarrhea,summarize the classic prescriptions for the treatment of diarrhea of liver depression and spleen deficiency.Results:Diarrhea of liver depression and spleen deficiency is a common syndrome described in ancient literature.Traditional Chinese medicine has a long history and rich experience in treating diarrhea of liver depression and spleen deficiency.Tongxie Yaofang is a classic prescription for treating diarrhea of liver-stagnation and spleen-deficiency.At the same time,there are clinically different types of liver-stagnation and spleen-deficiency.Traditional prescriptions include Sini San,Xiao Chaihu Decoction,Xiaoyao San,Xiaojianzhong Decoction and so on.Conclusion: Treating diarrhea due to liver depression and spleen deficiency from the theory of liver and spleen has profound theoretical origins.The second department:Shugan Jianpi Recipe mediated Ca MKⅡ/c-fos pathway to reduce visceral sensitivity and prevent diarrhea-type irritable bowel syndrome Objective:To explore the mechanism of Shugan Jianpi Recipe mediated Ca MKⅡ/c-fos pathway to reduce visceral sensitivity and prevent diarrhea-type irritable bowel syndrome.Methods:Ninety clean-grade male SD rats were randomly divided into normal,model,low,medium and high doses of Shugan Jianpi Decoction and control group according to their body weight.Except for the normal group,all the other groups used restraint-tail-clamping stress combined with capsaicin to prepare IBS-D models.All rats were sacrificed after the last administration,and the hypothalamus,pituitary and colon tissues were removed.Light microscope was used to detect the histopathological changes in the colon of rats with IBS;Real-Time PCR and Western-blot were used to detect the expression of P-Ca MKⅡ,c-fos m RNA and protein in each group.Results:Under light microscope,the colonic mucosa of each group was intact and the mucosal epithelium villi were arranged properly,which conformed to the IBS model performance.The expression of P-Ca MKⅡ and c-fos in the model group was significantly higher than that in the treatment group.After the intervention of Shugan Jianpi Recipe,the expressions of Ca MKⅡ and c-fos m RNA and protein in the treatment group were down-regulated compared with the model group;Among them,the expression of Ca MKⅡm RNA in the hypothalamus and the low and medium dose groups and the high-dose colon group,the expression of c-fos m RNA in the pituitary middle-,high-dose and high-dose colon groups,the expression of P-Ca MKⅡ protein in the hypothalamic low-dose group was poor,and the hypothalamus The difference of c-fos protein expression in the middle-dose group was statistically significant(P <0.05).The other treatment groups had a downward trend,but the difference was not statistically significant(P> 0.05).Conclusion: Shugan Jianpi Recipe reduces the expression of Ca MKⅡ after phosphorylation,down-regulates the expression of c-fos,reduces visceral hypersensitivity,and exerts the effect of preventing and treating IBS.
