Study On The Medication Rule Of Traditional Chinese Medicine In The Treatment Of Diabetes Mellitus Complicated With Hyperuricemia Based On Data Mining

Objective: The number of patients with Diabetes Mellitus(DM)complicated with Hyperuricemia(HUA)increases year by year,and the two diseases are closely linked,which can increase the incidence of the other party.The risk of hypertension and coronary heart disease is higher than that of general DM patients,and the kidney damage is aggravated and the rise of blood lipid is more obvious.The two diseases have similar etiology and pathogenesis,both of which are aimed at dampness-heat stasis and phlegm-deficiency treatment.Western medicine mostly adopts symptomatic treatment,which is prone to some side effects,symptoms and indicators are easy to rebound after drug withdrawal.The treatment of traditional Chinese medicine does not forget to consolidate the foundation,and it has good curative effect combined with the syndrome differentiation and treatment of constitution.Therefore,this study searched and collected the literature of traditional Chinese medicine treating DM complicated with HUA from various major databases,and used data mining technology to explore the rules of traditional Chinese medicine medication and therapy drug combination,and explore the potential prescription,in order to provide theoretical basis and prescription experience for clinical diagnosis and treatment.Methods: By searching all the literature on the treatment of DM combined with HUA by traditional Chinese medicine from the establishment of major databases such as China National Knowledge Infrastructure and Wanfang database to April 2022,the literature was screened in strict accordance with the inclusion and exclusion criteria,the data was collected,the name of traditional Chinese medicine was standardized,and the standard database of traditional Chinese medicine for the treatment of DM combined with HUA was established.Components of the included literature were collected by Microsoft Excel 2016,and drug frequency,category,sex and taste normalization were calculated.IBM SPSS Modeler 18.0 was used to study the drug association rules and find the rules of compatibility of traditional Chinese medicine.IBM SPSS Statistics 26.0 software was used to complete the high-frequency drug cluster analysis and discover the combination of therapeutic drugs.Finally,a unified theoretical analysis and discussion are made on the excavated results.Results:(1)In this study,a total of 80 literatures were selected,and 85 prescriptions of traditional Chinese medicine were recorded,involving 146 flavors of traditional Chinese medicine,with a total frequency of 988 times.The 27 traditional Chinese medicines with a frequency of > 10 times were used as high-frequency Chinese medicines,including Alisma Orientalis,Achyranthes bidentata,rhizoma dioscoreae colletii,Smilax glabra Roxb,Semen Coicis,Salvia miltiorrhiza,Glycyrrhiza uralensis,Cortex Phellodendri,Poria cocos,Radix astragali and other drugs.The 146 traditional Chinese medicines can be divided into 18 categories,of which the total frequency of use of heat-clearing drugs is the highest,followed by deficiency-tonifying drugs,water-disinhibiting damp-percolating medicinal,activating microcirculation and removing stasis medicinal herbs,and wind-damp dispelling drugs.The medicinal properties are in descending order of cold,flat,warm,cool and hot.The medicinal tastes are mainly sweet,bitter,xin,light and sour,and the main channels are liver,spleen,stomach,kidney and lung.(2)Association rule analysis : By setting the minimum rule confidence,the minimum conditional support,and the maximum number of preceding items,the association rules of two-flavor,three-flavor,and four-flavor Chinese medicine combinations are obtained.Among them,there are 11 pairs of association rules for two traditional Chinese medicine combinations,and the top four in support are : Cortex Phellodendri and Achyranthes bidentata;Clematis chinensis Osbeck and Achyranthes bidentata;Clematis chinensis Osbeck and rhizoma dioscoreae colletii;Codonopsis pilosula and Atractylodes macrocephala koidz.There are 39 pairs of association rules for the combination of three traditional Chinese medicines,and the top four are :Clematis chinensis Osbeck,rhizoma dioscoreae colletii and Smilax glabra Roxb;Clematis chinensis Osbeck,Achyranthes bidentata and rhizoma dioscoreae colletii;Clematis chinensis Osbeck,rhizoma dioscoreae colletii and Achyranthes bidentata;Atractylodes lancea,Cortex Phellodendri and Achyranthes bidentata.There are 14 pairs of association rules for the combination of four traditional Chinese medicines.The top four in support are : Atractylodes lancea,Cortex Phellodendri,Achyranthes bidentata and Semen Coicis;Semen Coicis,Smilax glabra Roxb,rhizoma dioscoreae colletii and Achyranthes bidentata;Semen Coicis,Achyranthes bidentata,rhizoma dioscoreae colletii and Smilax glabra Roxb;Clematis chinensis Osbeck,Smilax glabra Roxb,rhizoma dioscoreae colletii and Achyranthes bidentata.(3)Cluster analysis : high-frequency drugs can be clustered into 4 groups of traditional Chinese medicine,the first group : Atractylodes macrocephala koidz,Codonopsis pilosula,Poria cocos,Radix Rehmanniae Preparata,Cornus officinalis,semen persicae,Alisma Orientalis,Radix astragali,Safflower,Rhubarb.The second group : Glycyrrhiza uralensis,rhizoma coptidis,Radix Rehmanniae,Radix Puerariae,Salvia miltiorrhiza.The third group : Angelica sinensis,radix paeoniae alba,ginseng.Group 4 : Cortex Phellodendri,Atractylodes lancea,Achyranthes bidentata,Smilax glabra Roxb,Semen Coicis,rhizoma dioscoreae colletii,Clematis chinensis Osbeck,Semen Plantaginis,Cremastra appendiculata.Conclusions:(1)The most commonly used high frequency drugs in the clinical treatment of DM complicated with HUA include Alisma Orientalis,Achyranthes bidentata,rhizoma dioscoreae colletii,Smilax glabra Roxb,Semen Coicis,Salvia miltiorrhiza,Glycyrrhiza uralensis,Cortex Phellodendri,etc.The medicinal properties are mainly cold drugs,flat drugs and warm drugs.The flavors are mainly sweet drugs,bitter drugs and pungent drugs.The channels of drugs are most commonly used to enter the liver,spleen,stomach and kidney channels.In the category of drug use,the total use frequency of heat-clearing drugs ranks first,and the efficacy of other drugs focuses on tonifying deficiency,promoting water and dampness,promoting blood circulation and removing blood stasis.(2)The results of association analysis indicated that most of the drug combination was the combination of Clematis chinensis Osbeck,rhizoma dioscoreae colletii,Achyranthes bidentata,Cortex Phellodendri,Smilax glabra Roxb,which had the effects of clearing heat and detoxifying,dampness and phlegm,activating blood circulation and removing blood stasis,and tonifying deficiency,which was in line with the essence of the disease caused by the deficiency of the body and the stagnation of dampness,heat,blood stasis and phlegm in most DM patients with HUA.(3)The new prescription combination obtained by cluster analysis includes Atractylodes macrocephala koidz,Codonopsis pilosula,Achyranthes bidentata,Alisma Orientalis,rhizoma dioscoreae colletii,Smilax glabra Roxb,Cortex Phellodendri,semen persicae,safflower,Cremastra appendiculata,radix paeoniae alba and other drugs,which reflected that the clinical treatment of DM combined with HUA should take "tonifying spleen and kidney,clearing heat and dampness,removing blood stasis and eliminating phlegm" as the main treatment principles,and treat both deficiency and substance,so as to achieve the function of treating both symptoms and root causes.Taking into account the pathogenesis characteristics of DM complicated with HUA,such as dampness and heat of liver and stomach as the standard,weakness of spleen and kidney as the basis,mixed with dampness and heat,blood stasis and phlegm turbidity,it provides a new treatment idea and relevant medication reference for the clinical diagnosis and treatment of DM complicated with HUA.
