| Objective:The purpose of this study was to investigate the characteristics of defense mechanisms and coping styles in early adult patients with OCD and their correlation with the degree of compulsion,to verify the mediation mechanism of defense mechanisms between compulsion and coping styles,and to explore the effects and efficacy mechanisms of group persuation therapy on defense mechanisms and coping styles in patients with OCD.Methods:In Study 1,the characteristics of defense mechanisms and coping styles of patients with OCD were investigated and analyzed by questionnaire,and a mediation model was constructed.Sixtyone patients aged 18-25 years with diagnosed OCD in hospitals were selected to enter the patient group,and 57 healthy individuals aged 18-25 years were screened to enter the normal control group using the Symptom Self-Rating Scale(SCL-90)and the Yale-Brown Obsessive-Compulsive Scale(Y-BOCS).The assessors collected basic information about the patients and assessed the degree of symptoms,administered questionnaire measures to both groups using the Defense Mechanism Questionnaire(DSQ)and the Simple Coping Style Questionnaire(SCSQ)to assess the subjects’ psychological functioning in a self-assessment manner,derived data,and used SPSS 23.0 to perform descriptive statistics,t-test,chi-square test,pearson correlation analysis,and regression analysis on the questionnaire results processing.In Study 2,based on Study 1,the principle of convenience sampling was used to exclude the influence of other forms of treatment,and patients with OCD who received only group persuation therapy were recruited into the intervention group,and patients with OCD who did not receive any psychotherapy were recruited into the control group,and the final valid subjects:30 cases in the intervention group and 30 cases in the control group.The intervention group was given a 7-day structured closed group counseling treatment;the control group did not receive any psychological intervention.Questionnaires were administered to the subjects in both groups before and one year after the intervention,and the differences between the before and after measurements changes were tested for significance using SPSS23.0.On this basis,one typical case was selected and the efficacy study of group persuation therapy was supplemented by an in-depth observation of the study subjects and the group,as well as an interview one year after the intervention,combined with autobiographical descriptions of the condition and feedback materials during the group persuation therapy,using a narrative study method.Results:(1)Significant differences were found between the obsessive-compulsive disorder group and normal controls in scores on all dimensions of defense mechanisms and all dimensions of coping styles.(2)There was a significant correlation between mature and immature defense mechanisms and the degree of compulsion in early adult OCD patients,a significant correlation between coping style and the degree of compulsion,and a significant correlation between mature and immature defense mechanisms.(3)Coping tendency had a significant negative effect on the degree of compulsion,the coping style had a significant negative effect on immature defense mechanisms,and immature defense mechanisms had a significant positive effect on the degree of compulsion,while the effect of coping tendency on the degree of compulsion remained significant.Therefore,immature defense mechanisms partially mediated the effect between coping style and compulsion level,and the mediation effect was significant.(4)The mediating effect of mature defense mechanisms between coping style and compulsion level was not significant.(5)Subjects in the intervention group showed significant differences in the scores of immature and mature defense mechanisms in the Y-BOCS,SCL-90,SCSQ,and DSQ before and after one year of group facilitation treatment,while the differences in the scores of intermediate defense mechanism dimensions in the DSQ were not significant.(6)There were changes in the results of Y-BOCS,SCL-90,SCSQ,and DSQ measured before O and after one year in the control group,but the differences were not significant.(7)After the group psychological counseling treatment,there were significant differences in the degree of compulsion,mental health,defense mechanisms and coping styles measured before and after the intervention group,and the differences were significant with the control group measured before and after.(8)The analysis of typical cases revealed that there were dramatic changes in obsessivecompulsive symptoms,personality,coping styles,and defense mechanisms within one year after the end of group psychological guidance treatment.Conclusion:The coping style is the individual’s usual cognitive-behavioral style,and the psychological defense mechanism is the self-protective function,and there is a significant correlation between the three and the degree of compulsion.The efficacy of group persuation therapy is closely related to the patient’s psychological functioning,the theory of persuation therapy,the treatment setting,the leader,the presence of the group,and the living environment.Psychological persuation therapy deserves to be promoted and applied. |
