ObjectiveWith the continuous development of military informatization and modernization,the task of preparing for militarization and demilitarization is more arduous,and the combat form of future wars tends to be joint operations and local high-tech weapon operations,which makes war wounds and injuries show a high lethal intensity.The characteristics of long injury time,complex casualty mechanism,and increase of new injuries and new injuries have increased the scope and difficulty of medical treatment for the wounded,and put forward higher requirements for the medical support capability of military hospitals.As the backbone of the Air Force and even the entire military’s medical support system,the Air Force Hospital is responsible for the diagnosis and treatment of the wounded and sick,as well as medical treatment,evacuation,command and coordination in future operations.Nursing staff,as an important part of the Air Force Hospital,have assumed important responsibilities in tasks such as medical support,disaster care,public health emergency rescue,and air transport evacuation.Their job competency directly affects the guarantee and rescue effect.At present,the research on the competency of Air Force hospital nurses is still in the exploratory stage,mainly focusing on non-war military operations and military exercises.Standards research is insufficient.Therefore,the aim of this study is to construct a post competency model of the Air Force hospital nursing staff,and establish a competency standard that meets the needs of the post.Provide the basis for the training system of war wounded ambulance.MethodsLiterature research and questionnaire survey were used to preliminarily formulate the competency factors of nurses in the Air Force Hospital.We Searched on domestic and foreign databases,screened out 11 representative competency models or standards about competency of military nurses and military medical support related to Air Force,and then extracted competency elements.Using the self-made questionnaire that is aimed at understanding the education and training demand of Air Force hospital nurses to investigate affiliated hospitals of Air Force medical university and Air Force hospitals in the war zones.To understand their training needs,and develop a set of competency elements for hospital nurses in the Air Force Hospital according to the results of literature research.According to the above-mentioned competency factors,a questionnaire for the expert on competency indicators of nursing staff in the Air Force hospital was formed.29 experts in the field were invited to conduct two rounds of inquiries by e-mail,and screened out competency indicators according to the average value,coefficient of variation,expert recognition rate,and preliminarily established competency model.According to the results of Delphi methods,a self-assessment questionnaire(trial)about the competency of nurses in Air Force hospital was formed.After 2 rounds of questionnaires,item analysis and factor analysis were used to screen items.Then we established the competency model of the Air Force hospital nurses,and explored the competency structure.A survey was conducted using a formal questionnaire,and reliability and validity tests were performed in order to validate and optimize the model.Finally,using One-ANOVA and multiple linear regression analysis to comprehend the current competency status and affecting factors of civilian nurses in the Air Force hospitals.ResultsLiterature research and questionnaire survey proposed 23 competency elements,and after discussion by the group,a questionnaire for experts on the competency of Air Force hospital nurses with 3 modules with a total of 104 items was formed,including military nursing competency,clinical nursing competency,and professional development competency.After 2 rounds of expert correspondence,20 indicators were deleted,and a preliminary model with 84 items was formed.529 and 318 valid questionnaires were collected in the 2 rounds of questionnaires,and were screened by item analysis and exploratory factor analysis on the three modules of military nursing ability,clinical nursing ability,and professional development ability.13 entries.Structural exploration suggested that 9 common factors should be extracted,and they were named as military and political literacy,ambulance technology,air evacuation,acute and critical care,nursing humanities,basic nursing management,critical thinking,communication and collaboration,and personal characteristics.Established a competency model of Air Force hospital nurses including 49 indicators.In the model validation stage,1585 valid questionnaires were recovered,618 were selected for reliability and validity test,218 were used for exploratory factor analysis,and400 were used for confirmatory factor analysis.The Cronbach’s alpha was between 0.899and 0.973,all>0.8.The split-half reliability coefficient was between 0.794 and 0.939,all>0.7.The overall fitting results of the model:X~2/df=2.630,RMSEA=0.064,CFI=0.913,TLI=0.906,and they all meet the statistical requirements.Convergent validity results:AVE values ranged from 0.5096 to 0.8053,CR combined reliability ranged from0.8778 to 0.9680,and each factor loading coefficient was>0.7,indicating good convergent validity.The discriminant validity showed that the value of Square Roof of AVE should was higher that the correlation.The reliability and validity test show that there is good reliability and validity.Competency status shows that the Air Force hospital nurses have a medium level of competency,a higher score in military and political literacy,personal characteristics,communication and collaboration,and a lower score in critical thinking,air evacuation,and ambulance skills.The multivariate linear stepwise analysis results of the total competency score and the three competency modules show that age,whether they have participated in on-the-job education,professional title,unit location,field medical team member,personnel category,marriage entry model and the regression coefficient are statistically significant.The dependent variables are different,and the influencing factors are also differentConclusionsThe competency model of Air Force hospital nurses evaluates the competency of nurses from 3 modules and 9 dimensions:military nursing competency,clinical nursing competency,and professional development competency.The results display that the model has good reliability.It can provide reference and basis for the self-assessment of nursing staff and the development of training and education plans for nursing staff.
