| Objective To compare the efficacy of functional expansion pharyngoplasty and uvulopalatopharyngoplasty in the treatment of patients with severe obstructive sleep apnea hypopnea syndrome.Methods The patients admitted to the Otolaryngology Head and Neck Surgery of Loudi Hospital Affiliated to South China University from January 2020 to June 2021 were selected for effective polysomnography monitoring for more than 7 hours,suggesting that apnea hypopnea index(AHI)> 30 times/h and met the inclusion and exclusion criteria.According to the voluntary principle,those who underwent uvulopalatopharyngoplasty were divided into the UPPP group(n = 23),and those who underwent functional expansion pharyngoplasty were divided into the FEP group(n = 35).To observe the anterior and posterior pharyngeal diameters,healing rate and complications of the two groups,we compared the lowest pulse oxygen saturation(LSp O2),AHI,snoring score(SS)and drowsiness score(DS)in the two groups in June.Results1.There was no statistical significance in the comparison between the two groups of patients in terms of age,sex and apnea hypopnea index(P>0.05).2.There was no statistical significance in comparing the anterior and posterior pharyngeal diameters or the left and right diameters of the two groups of patients before surgery(P>0.05);The front and posterior and right diameters of the pharyngeal cavity after FEP group were larger than those in the UPPP group,and the difference between the two groups is statistically significant(P<0.05).3.Lspo2,AHI,SS and DS were improved in both groups 6 months after the operation.The postoperative AHI and SS in the FEP group were significantly less than those in the UPPP group(P<0.05).4.The healing rate was 82.9% in FEP group and 47.8% in UPPP group.Compared with UPPP group,FEP group had higher healing rate(P<0.05).5.Among 35 patients in the FEP group,2 patients had complications,with an incidence of 5.7%.Among 23 patients in the UPPP group,8patients had postoperative complications,with an incidence of 34.8%.The probability of postoperative complications in the FEP group was significantly lower than that in the UPPP group(P>0.05).ConclusionIn patients with severe obstructive sleep apnea hypopnea syndrome,FEP has fewer postoperative complications and healing rate than UPPP,which is worthy of clinical promotion. |
