| Purpose:This article aims to investigate the expression of MALT1in patients with different types of diffuse large B-cell lymphoma, and to study the effect of MALT1 expression on the clinical features and prognosis of GCB-DLBCL and ABC-DLBCL.Method:The case data of 67 newly diagnosed DLBCL patients admitted to our hospital from June 2017 to February 2022 were retrospectively analyzed, and the expression of MALT1 was detected by immunohistochemistry(IHC).SPSS 23.0 statistical software was used to analyze the effect of MALT1 expression on the clinical characteristics and prognosis of different types of DLBCL.In all statistical analyses,P<0.05was considered statistically significant.Result:1.The age of onset of 67 DLBCL patients was between 28and 84 years old,and the median age of onset was 65 years old; There were36 cases(53.7%)aged>60 years, and 31 cases(46.3%)aged≤60years;There were 40 male patients(59.7%)and 27 female patients(40.3%);There were 39 cases(58.2%)with normal LDH,and 28 cases(41.8%)with increased LDH;There were 32 cases(47.8%)with stage I-II,and 35 cases(52.2%)with stage III-IV;There were 38 cases(56.7%)with extranodal involvement, and 29 cases(43.3%)without extranodal involvement; There were 35 cases(52.2%)with ECOG≥2,and 32 cases(47.8%)with ECOG<2;There were 32 cases(47.8%)with increasedβ2-MG,and 35 cases(52.2%)with normalβ2-MG;A total of 35 cases(52.2%)had B symptoms, and 32 cases(47.8%)had A symptoms.2.Among the 67 newly diagnosed DLBCL cases,27(40.3%)were ABC subtype,and40(59.7%)were GCB subtype.Among the ABC subtypes, there were 17cases(63.0%)aged≤60 years, and 10 cases(37.0%)aged>60years;Among the GCB subtypes, there were 15 patients(35.0%)aged≤60years old,and 23 patients(65.0%)aged>60 years old. There was a statistically significant difference in age distribution between the two groups(P=0.024).There was no significant difference in LDH,β2-MG,gender,B symptoms, stage,IPI score and ECOG score between the two groups(P>0.05).3.IHC results showed that the expression of MALT1 was located in the cytoplasm, which was brown.4.From June 2017 to February2020,a total of 42 patients were initially diagnosed with DLBCL in the Department of Hematology of our hospital and received regular chemotherapy, including 15 patients with ABC subtype and 27 patients with GCB subtype.Among them,MALT1 is in the ABC subtype. The expression in patients with type 2 was all positive;Seventeen(63.0%)of the GCB subtype patients had positive expression, and the remaining 10(27.0%)had negative expression. There was a statistically significant difference in the expression of MALT1 between ABC subtype and GCB subtype patients receiving treatment(P<0.05).5.There was no significant correlation between MALT1 and LDH,β2-MG and stage of DLBCL patients.6.The survival time of 42 patients who received treatment ranged from 1 to 55 months. In the treatment group, the 2-year survival rate of patients with ABC subtype was 100%,and the 2-year survival rate of patients with GCB subtype was 93.5%,and the difference was not statistically significant.(p=0.394).Among the treated GCB subtypes,the2-year survival rate of MALT1 positive expression was 92%,and the2-year survival rate of MALT1 negative expression was 100%,and the difference was not statistically significant.Conclusion:1.Among DLBCL patients, the age of onset of ABC subtype tends to be younger, while GCB subtype is more common in the elderly.2.The positive expression rate of MALT1 in DLBCL patients was76.1%,and the positive rate of ABC subtype was significantly higher than that of GCB subtype, and the difference was statistically significant.3.The positive expression of MALT1 has no correlation with LDH,β2-MG and stage.4.In the treated GCB subtypes, the survival time of MALT1 negative expression may be longer than that of positive expression(the difference is not statistically significant). |
