Objectives Understand the current status of the core competence,actual core competency requirements,suggestions and expectations for the curriculum of emergency pre-examination and triage nurses in Xi’an public general hospitals,and build a core competency-based emergency pre-examination and triage nurse training curriculum to improve emergency pre-examination scores.Provide systematic and scientific theoretical basis and practical guidelines for the core competence of nurses to train emergency pre-examination and triage nurses who are adapted to their positions.Methods1.A total of 295 emergency pre-examination and triage nurses were selected from 19level-III public general hospitals and 14 level-II public general hospitals from August to December 2019.A self-designed questionnaire on the core competence of emergency pre-examination and triage nurses was used to conduct a questionnaire survey.The composition ratio and mean ± standard deviation were used to describe the status quo of emergency pre-examination and triage positions,basic information,working conditions,qualifications and training,assessment and core competence scores of emergency pre-examination and triage nurses in Xi ’an Public General Hospital.T-test,analysis of variance and multiple linear regression were used to explore the influencing factors of core competence of nurses in pre-examination and triage in emergency department.2.The objective sampling method was used to form an interview outline on the basis of current situation investigation and literature analysis.The semi-structured interviews were conducted among 4 emergency pre-examination and triage nurses and 6 emergency nursing experts in Xi ’an Public General Hospital.3.On the basis of the status quo survey and qualitative interviews,preliminary to formulate emergency preview triage nurse training curriculum framework and entry,invited as xi ’an nursing association professional committee of emergency nursing expert argumentation and modification of the curriculum of the first round of experts enquiry for questionnaire and distribute to 24 emergency related experts,after two rounds of Delphi expert consultation,finally,the curriculum setting of emergency pre-examination and triage nurses training based on core competence was established,including course modules,course objectives,core courses,course contents,reference hours,teaching forms,and assessment methods.Results1.Investigation results of core competence of emergency pre-test and triage nurses in Xi’an public general hospitalA total of 295 questionnaires were sent out and 256 valid questionnaires were received with an effective recovery rate of 86.8%.The total score of core competence of emergency pre-test and triage nurses in Xi ’an Public General Hospital was(208.45±26.23)on average,and the average score of six dimensions were,from high to low,management ability,critical thinking ability,communication and coordination ability,professional practice ability,pre-test and triage ability,and professional development ability.Among them,the average score of professional development ability dimension(4.06±0.70)was lower than the average score of total items(4.25±0.54).Multiple linear regression analysis showed that age,professional title,belief that they are capable of accurate triage,and relevant training before triage were the main influencing factors of core competence of nurses in emergency pre-test and triage.2.Qualitative interview results of core competence training needs of emergency pre-examination and triage nursesFor emergency care experts and emergency preview triage nurse for interviews,respondents agree that emergency preview triage nurse assumes the role of multiple,according to the different role positioning,need for six core competence,namely,management ability,critical thinking ability,communication and coordination ability,professional practice ability,engineering ability of triage,professional development.It is not for the six core ability training courses,and the existing about preview triage of one-sided training content,training quality,the core competence of the need for the above six emergency preview triage training courses,and the core curriculum modules,curriculum objectives,curriculum,course content,etc.Some ideas and Suggestions are put forward.3.Results of training curriculum for nurses in emergency pre-examination and triage based on core competence3.1 The effective recovery rate of the two rounds of expert letter questionnaire was 100%,the expert opinion submission rate was 79.2% and 54.2% respectively,and the expert authority coefficient was 0.914.The mean score of importance,standard deviation and full mark ratio of the first round of expert letter consultation indexes were 3.8~5.0,0.00~1.16 and 33.3%~100.0%,respectively.The coefficient of variation was 0.00%~29.00%,and the expert coordination coefficient was 0.321(P < 0.05).The mean score of importance,standard deviation and full mark ratio of the second round of expert letter consultation indexes were 4.3 ~ 5.0,0.00 ~ 0.78,50.0% ~ 100.0%,respectively.The coefficient of variation was 0.00%~18.00%,and the expert coordination coefficient was0.334(P<0.05).3.2 The training course for emergency pre-examination and triage nurses based on core competence is composed of seven parts: course module,course objective,core course,course content,teaching method,reference class hours,and assessment method.The course module includes six modules: pre-test and triage ability,communication and coordination ability,critical thinking ability,professional practice ability,management ability,and professional development ability.There are 32 items in the course objectives;Preview of core curriculum is equipped with emergency preview triage and emergency treatment triage interpersonal communication,using of scientific thinking in clinical nursing decision making,critical patients nursing,health education,nursing related laws and regulations,the hospital infection control and occupational protection,emergency triage management,emergency triage nurses professional quality,emergency triage nurses,nursing research,nursing career planning education,mental health dimension13 courses of nursing;The course content is a refinement of the core curriculum,with a total of 46 items.The course consists of 113.5 reference hours;Theory teaching,case analysis,scenario simulation,role play,group discussion,special lectures and other teaching methods;Assessment methods according to the course content characteristics of the written test and practice.Conclusions1.In Xi ’an Public General Hospital,emergency pre-examination and triage posts are not paid attention to.The proportion of primary professional titles of emergency pre-examination and triage nurses is high,and there are many part-time nurses.The auxiliary triage equipment lacks emergency protection,and the information technology is not fully popularized,so more efforts should be made to build and develop.2.The existing triage training has weak pertinence,uneven training quality and lack of standardized training courses,so the optimization and standardization of triage training is imminent.3.The core competence of nurses in emergency pre-examination and triage in Xi ’an public general hospital is not high,and the ability of pre-examination and triage and professional development are weak.Therefore,targeted measures should be taken from the aspects of professional title and pre-triage training to improve the core competence of nurses in emergency pre-examination and triage.4.Based on the core competence of emergency preview triage nurse training program,guided by the demands of jobs,in the framework of core competence,including core modules,curriculum goals,course content,has not only theoretical basis,and has the realistic foundation,has the certain scientific and practical,for the subsequent conduct emergency preview triage specialist training.To provide reference basis for improving the core competence level of nurses.
