| Objective: This research was designed to investigate the effect of thyroid stimulating hormone(TSH)suppression therapy on postoperative recurrence and metastasis of differentiated thyroid carcinoma(DTC)in the elderly.Methods: Totally,we enrolled 110 patients,who were over 60 years old DTC with a concurrent radical thyroidectomy,by retrospective analysis from June 2015 to December 2017 in our hospital.Among them,55 cases were treated with TSH suppression therapy(suppression group)and 55 cases were treated with thyroid replacement therapy(replacement group).Results: No significant difference was observed in the basic data between the two groups(P>0.05);in this study,the 3-year distant recurrence rate of the inhibition group was significantly lower than that of the replacement group(1.82% VS 14.55%),The rate of patients with lymph node metastasis was 3.64%,significantly lower than 20.00%in the replacement group;the probability of distant metastasis in the inhibition group was significantly lower than that in the replacement group(1.82% VS 16.36%),the differences were statistically significant(P<0.05).Seven days before operation,thyroid hormone levels of the two groups were compared.The results showed that there was no significant difference in TSH(2.57±0.28),FT4(17.03±0.74),FT3(4.92±0.06)between the replacement group and the inhibition group(2.64±0.16),FT4(17.10±0.69),FT3(4.90±0.07)7 days before operation(P>0.05).After 3 years of follow-up,TSH(2.63±0.34)of replacement group was significantly higher than that of inhibition group(0.24±0.07),FT4(18.03±0.74)was significantly lower than that of inhibition group(23.10±0.69),FT3(5.25±0.06)was significantly lower than that of inhibition group(6.75±0.07),the difference was statistically significant(P<0.05).Conclusion: For elderly DTC patients,TSH inhibitory therapy has high clinical application value and has the effect of reducing recurrence and metastasis.It can be regarded as an effective drug treatment program for elderly DTC patients after surgery. |
