| Healthy arteries are elastic,but with mechanical damage caused by long-term hypertensive blood flow impact,physical and chemical factors,such as inflammation factors and oxidation factors,stimulation and destruction,and lipid deposition,the vascular wall fibrous tissue hyperplasia and calcification,and relieve headaches and create plaque.With the change of people’s lifestyle and the aging of population,the incidence of arteriosclerosis and its complications shows an obvious increasing trend.Arteriosclerosis is not only one of the most common vascular diseases,but also the most common cause of cardiovascular and cerebrovascular diseases and peripheral vascular diseases.Serum Fibroblast growth factor 21(FGF21)is a critical metabolic regulatory factor in human body,and plays an important role in the protection of atherosclerosis and a variety of cardiovascular and cerebrovascular diseases.ObjectiveIn this study,by comparing the relationship between serum fibroblast growth factor 21(FGF21)expression level and brachial and ankle pulse wave conduction velocity(ba PWV)in patients with grade 3 hypertension combined with arteriosclerosis,the correlation between FGF21 and the degree of arteriosclerosis was analyzed,providing evidence for the prevention and treatment of cardiovascular diseases.MethodsPatients with hypertension admitted to the Second Affiliated Hospital of Shenyang Medical College from September 2021 to December 2022 were selected.Patients with a mean value of ba PWV ≥14m/s were selected as the experimental group.According to ba PWV value,the test groups were divided into mild atherosclerosis group(14m/s≦ba PWVm/s),moderate atherosclerosis group(16m/s≦ba PWVm/s)and severe atherosclerosis group(ba PWVm/s).Healthy people in the same period(no hypertension,Bilateral ba PWV < 14m/s)was the control group,which was divided into four groups.General data and related clinical indicators of patients were collected,serum FGF21 content of the enrolled patients was measured,and the relationship between FGF21 and ba PWV among all groups was analyzed and compared,so as to infer the diagnostic effect of FGF21 on the degree of arteriosclerosis.Results1.Comparison of general data and laboratory indexes of arteriosclerosis group,subgroups and control group.There were significant differences in gender,age,BMI,fasting blood glucose,WBC,homocysteine,TG,TC,HDL-C,LDL-C,AIP,FGF21 and ba PWV between the arteriosclerosis group and the control group(P<0.05).There was no significant difference in smoking and drinking between the atherosclerosis group and the control group(P>0.05).Comparison between groups: There were significant differences in age,BMI,fasting blood glucose,WBC,homocysteine,TG,TC,AIP and FGF21 between the mild arteriosclerosis group and the control group(P<0.05).There were significant differences in age,BMI,fasting blood glucose,homocysteine,HDL-C,TG,AIP and FGF21 between the moderate sclerosis group and the control group(P<0.05).There were significant differences in age,BMI,fasting blood glucose,homocysteine,HDL-C,LDL-C,TG,TC,AIP and FGF21 between the severe atherosclerosis group and the control group(P<0.05).There was a significant difference in FGF21 between the mild and moderate atherosclerosis groups(P<0.05).There were significant differences in AIP and FGF21 between the mild atherosclerosis group and the severe atherosclerosis group(P<0.05).There were significant differences in LDL-C,TC and FGF21 between the severe atherosclerosis group and the moderate atherosclerosis group(P<0.05).2.Correlation analysis of general data and laboratory indexes with bilateral ba PWV in patients with atherosclerosis.Spearman linear correlation analysis showed that age,alcohol consumption,BMI,WBC,fasting blood glucose,homocysteine,TG,LDL-C,TC,AIP were positively correlated with the mean value of ba PWV,(P<0.05)and were highly positively correlated with age(r>0.7).HDL-C and FGF21 were negatively correlated with the mean value of ba PWV,and were moderately negatively correlated with FGF21(0.5<r<0.7).There was no correlation between smoking and sex and the mean ba PWV(P>0.05).3.The factors affecting the concentration of FGF21 were analyzed by linear regression.The factors influencing FGF21 concentration were analyzed by multiple stepwise linear regression,and the mean value of ba PWV was an independent predictor of FGF21(P<0.05).4.logstic regression analysis of risk factors for arteriosclerosis and its severity.The results of binary logstic regression analysis showed that BMI,TC,fasting glucose and AIP were independent risk factors for atherosclerosis(P<0.05).HDL-C and FGF21 were independent protective factors for arteriosclerosis(P<0.05).The results of multiple ordered logstic regression analysis showed that FGF21 was an independent protective factor and AIP was an independent risk factor for the severity of arteriosclerosis(P<0.05).5.The diagnostic value of FGF21 and AIP in arteriosclerosis was studied by ROC curve.ROC curve results showed that compared with the control group,the area under the curve(AUC)of serum FGF21 in the arteriosclerosis group was 0.868(95%CI:0.808-0.927,P=0.000),the cut-off value was 225.72pg/ml,and the sensitivity was:0.637,the specificity was 0.969,the Yoden index was 0.606.The AUC value of FGF21 in detecting arteriosclerosis was between 0.7 and 0.9,indicating that it had certain accuracy.Conclusions1.Plasma FGF21 level was significantly decreased in patients with arteriosclerosis,and was negatively correlated with the severity of arteriosclerosis.2.FGF21 is an independent predictor of arteriosclerosis and has good predictive value for the severity of arteriosclerosis. |
