[Objective]The purpose of the study was to observe the efficacy of traditional Chinese spinal chiropractic manipulation(TCSOM)in treating female stress urinary incontinence(SUI),and to compare the efficacy to that of Kegel exercise as the control group.[Methods]From 2019 to 2022,60 female SUI patients were recruited from the Department of Traditional Chinese Orthopedics and Traumatology of Zhongda Hospital affiliated to Southeast University and Xinminmen clinics in Gulou,Nanjing.The patients were randomly divided into a study group and a control group,30 cases in each group.The study group was treated with TCSOM,3 times a week,for a total of 4 weeks;the control group was treated with Kegel training,once a day,for 4 weeks.Before the first treatment and after 4 weeks of treatment,the ICI-Q-SF score and I-QOL score of the patients were recorded,and the curative effect was determined according to the improvement rate of the ICI-Q-SF score,and the adverse reactions in the two groups were recorded.SPSS24.0 was used for data processing and analysis,t test was used to analyze measurement data,and chi-square test was used to analyze count data,the test level was α=0.05.[Results]The research subjects in the TCSOM group and the Kegel group were all women.The age range of 30 patients in the TCSOM group was 40-70 years old,with an average age of 55.17±4.13 years,and the age range of 30 patients in the Kegel group was 38-70 years old,with an average age of 54.77±4.07 years old.The disease duration of the patients in the TCSOM group ranged from 1 to 20 years,with an average of 8.57±1.77,and the disease duration of the patients in the Kegel group ranged from 1 to 20 years,with an average of 8.37±1.75.There were no significant differences in gender,age,course of disease,disease severity and other components between the two groups(P>0.05).All 60 patients completed the 4-week treatment regimen.After 4 weeks of treatment,the ICI-Q-SF score of the TCSOM group was lower than that of the Kegel group(P<0.05),and the I-QOL score was higher than that of the Kegel group(P<0.05).Based on the improvement rate of ICI-Q-SF score,in the TCSOM group,8 cases were cured,12 cases were markedly effective,5 cases were effective,5 cases were ineffective,and the effective rate was 83.3%;in the Kegel group,3 cases were cured,and 5 cases were markedly effective,9 cases were effective,13 cases were ineffective,and the effective rate was 56.7%.There was a significant difference in the effective rate between the two groups(P<0.05).The improvement of symptoms in the TCSOM group was significantly higher than that in the Kegel group.There were no adverse reactions in the two group.[Conclusion]TCSOM is effective in treating female SUI,effectively relieving lumbar discomfort,reducing the number and frequency of urinary leaks,decreasing the volume of urinary leaks,and improving quality of life.Treatment with acupressure and lumbar spine positioning oblique trigger method at the lumbar 2 to lumbar 3and/or lumbar 4 to sacral 1 segments can correct intervertebral displacement,relieve the compression and irritation of lumbosacral nerves due to intervertebral displacement,improve the internal and external balance of the lumbosacral spinal canal,and thus reduce or eliminate the symptoms of urinary incontinence.Therefore,TCSOM can correct and improve the lumbosacral spine or intervertebral disc displacement,restore the imbalance of yin and yang inside and outside the lumbosacral spinal canal,relieve the compression and stimulation of nerves and blood vessels due to lumbosacral spine displacement,balance the yin and yang of the lumbosacral spine,eliminate or improve the symptoms of SUI in women,and have the effect of strengthening the capital,tonifying the kidney and nourishing the qi,and tonifying the spleen and lifting the trap. |
