| Objective1.Using resting-state functional magnetic resonance imaging technology to study the changes of brain functional connectivity in herpes zoster patients,and to analyze the correlation between functional connectivity changes and clinical characteristics.2.To summarize the clinical data of patients with herpes zoster with cardiovascular and cerebrovascular complications,analyze the risk factors of complications,and explore the correlation between antiviral therapy and complications.Methods1.A total of 50 cases in the experimental group and 39 cases in the control group were selected,who were hospitalized in the Department of Dermatology,Zhongda Hospital from August 2017 to February 2022,and met the criteria.Using rs-fMRI to acquire the imaging date.Using seed-based functional connectivity analysis method to compare the difference between HZ patients and healthy controls.Analysing the correlation between imaging parameters of different brain regions and clinical features.2.Selected 23 HZ patients with cardiovascular and cerebrovascular complications and 44 HZ patients without cardiovascular and cerebrovascular complications who were hospitalized in Zhongda Hospital from January 2017 to January 2022.Statistical analysis of clinical data of two groups of patients.To study the association between antiviral therapy and complications.Results1.Seed-based functional connectivity changes in HZ patients.All statistical results were corrected by voxel-level P<0.001,cluster-level P<0.05 and FEW correction.Compared with the control group,the functional connectivity between middle frontal gyrus and right orbital middle frontal gyrus was enhanced in the patient group.The functional connectivity between thalamus and left superior temporal gyrus and left middle temporal gyrus were enhanced in the patient group.The functional connectivity between putamen and left parahippocampal gyrus was enhanced in the patient group.2.Correlation analysis between clinical data and FC values in HZ patient.The FC value of right orbital middle frontal gyrus and middle frontal gyrus was correlated with visual analogue scale in HZ patients(rs=-0.333,p=0.022).3.Risk factors analysis of herpes zoster with cardiovascular and cerebrovascular complications.Multifactorial logistic regression analysis showed that age was a risk factor for cardiovascular and cerebrovascular complications in HZ patients.For each additional year of age,the risk increased by nearly 0.148-fold.Conclusion1.Compared with healthy controls,HZ patients showed enhanced functional connectivity in multiple brain regions,which are significantly associated with pain,suggesting that neural mechanisms may play a crucial role in zoster-associated pain.2.The FC value of right orbital middle frontal gyrus and middle frontal gyrus in HZ patients might be a potential imaging marker of pain intensity.3.Elderly patients with herpes zoster are more prone to cardiovascular and cerebrovascular complications.Diabetes,hypertension,and lymphocyte ratio may be risk factors.There is no correlation between antiviral therapy and the occurrence of cardiovascular and cerebrovascular complications. |
