ObjectiveThrough a case-control study,to compare the disease progression and recovery of two groups of children with mixed Ig A vasculitis with a general diet and animal protein-free diet so as to avoid unnecessary diet restriction and affect the prognosis,growth and mental health of the children.New ideas for dietary therapy for children with Ig A vasculitis would be provided.MethodsA total of 58 children were first diagnosed with mixed Ig A vasculitis(both abdominal type and articular type)and hospitalized in our Hospital from October2021 to February 2023,After informed consent was obtained from the child and their parents,they were randomly divided into general diet group or animal protein-free group according to the random number table.Collect the data of two groups,such as sex,age,weight and height(on admission day,the 6th day of admission,and 4 weeks of hospital discharge),the days of hospitalization,the days of disappearance of skin rash,the days of improvement of abdominal pain,scores of the anxiety(on the day of admission and discharge).The two groups were collected blood for testing blood routine before medication on the day of admission,blood routine was reviewed on the day of hospital discharge,Neutrophil count/ lymphocyte count(NLR)and Platelet count / Lymphocyte count(PLR)were calculated.cleaned urine in the middle of the morning on the second day and of the fifth day admission from the two groups were tested.the blood routine,urine routine and the number of relapses at the second and fourth weeks of discharge were collected,and the height and weight at the fourth week of follow-up were recorded.The results of which including sex,age,weight,height,the days of hospitalization,the days of disappearance of skin rash,the days of improve-ment of abdominal pain,scores of the anxiety,blood routine,routine urine,number of recurrence at follow-up,they would be showed through SPSS22.0 software for conduct-ing the statistical analysis.The((?)±s)would be used if the data conform to the normal distribution with t test.If the data don’t conform to the normal distribution,the [M(P25,P75)] would be used with if the two independent samples from the non-parametric tests.enumeration data would be showed in a percentage rate(%),The χ 2 test would be used to the enumeration data.p <0.05 is statistically significant.Results1.Comparison of general data between the general diet group and animal protein-free diet group: there were 28 patients in the general diet group,accounting for 48% of the total sample size,including 17 male and 11 female cases,with an average age(89.35±29.55)months.there were 30 in patients animal protein-free diet group,accounting for 52% of the total sample size,16 male and 14 female,with mean age(95.90±27.67)months.There were no statistically significant in sex(χ 2=0.32,P> 0.05),age(t=-0.87,P> 0.05)of the children between the two groups(P> 0.05).2.Comparison of the basic condition of children on admission in the general diet group and animal protein-free diet group: the duration of the rash on admission in the two groups(days)[3.0(2.0,4.0)VS 3.0(2.0,4.0)],time of abdominal pain on admiss-ion(days)[2.0(1.0,2.0)VS 2.0(1.0,2.0)],time of joint pain on admission(days)[1.5(1.0,2.0)VS 2.0(1.0,2.0)].Basic condition status of children in the general diet group and animal protein-free diet group: duration of rash on admission(Z=-0.11,P>0.05),duration of abdominal pain on admission(Z=-0.12,P>0.05),duration of joint pain on admission(Z=-0.28,P>0.05),there were not statistically significant(P>0.05),It is comparable.3.Comparison of clinical observation indicators between the general diet group and animal protein-free diet group with the results that the days of hospitalization(8,25±2.06 VS 8.26±2.34),and rash disappearance(6.89±2.18 VS 7.50±1.76),and abdomin-al pain improvement[3.00(2.00,3.00)VS 3.00(3.00,3.25)],and disappearance of abdominal pain[4.00(3.25,5.00)VS 5.00(4.00,5.25)],there are no significant differenc-es(hospitalization days t=-0.43,P>0.05,Days when the rash disappeared t=0.41,P>0.05,Days of abdominal pain Z=-1.593,P>0.05,Days of abdominal pain Z=-1.39,P>0.05).comparison of clinical observation indicators between the general diet group and animal protein-free diet group with the results that the days of joint pain recovery[3.00(2.00,3.00)VS 3.00(3.00,3.25)],There was significantly different(Z=-2.43,P<0.05),the days of joint swelling in the general diet group were shorter than animal protein-free diet group.4.Comparison of blood routine(NLR and PLR)between the general diet group and animal protein-free diet group with the results that blood routine NLR on the day of admission [1.84(1.18,3.54)VS 1.75(1.33,2.99)],blood routine NLR on the day of discharge [1.40(0.81,2.54)VS 1.14(0.72,2.18)],blood routine PLR on the day of admission[105.92(83.50,158.98)VS 99.27(77.91,127.19)],blood routine PLR(109.86±44.32 VS 99.84±33.94)on the day of discharge in two groups,there were no significant difference.(on the day of admission Blood routine NLR Z=-0.01,P>0.05,on the day of discharge Blood routine,NLR Z =-1.05,P>0.05,on the day of admission Blood routine PLR Z=-0.66,P>0.05,on the day of discharge Blood routine PLR Z=0.97,P>0.05).5.Comparison of the increase of height and weight of the two groups in 6 days of admission and 4 weeks from discharge: the growth value of height in 6 days of admis-sion[0.1(0.1,0.2)VS [0.1(0.1,0.2)],the growth value of height in 4 weeks from discharge[0.36±0.12 VS 0.35±0.13],There was no significant difference.(Height growth values of the two groups in 6 days of admission Z=-0.24,P>0.05,Height growth values of the two groups in the 4 weeks from discharge t=0.45,P> 0.05).Weight growth values in 6 days of admission(-0.02±0.15 VS-0.11±0.16),Weight growth values in 4 weeks from discharge(0.23±0.14 VS 0.04±0.14),There was significantly different.(increase value of height of two groups in 6 days of admission t=2.18,P<0.05,increase value of weight of two groups in the 4 weeks from discharge t=5.09,P<0.05).The growth value of weight in 6 days from admission and 4 weeks from discharge that the general diet group were higher than the animal protein-free diet group.6.Comparison of scores of anxiety self-rating scale between the general diet group and animal protein-free diet group on the day of admission and discharge:the scores of anxiety self-rating scale on the day of admission(50.64±5.20 VS 51.23±5.15)in two groups,there are no significant differences(t=-0.83,P>0.05).The general diet group and animal protein-free diet group were in mild anxiety score(50-59 points).the scores of anxiety self-rating scale on the day of discharge(38.39±3.71 VS 41.10±4.52)in two groups,There was significantly different.(t=-0.37,P<0.05)the mean scores of both groups were normal,and the anxiety scale score of children in the the common diet group was lower than that of the animal protein-free diet group.7.During the discharge follow-up of children with mixed Ig A vasculitis,1 case in the general diet group had rash recurrence,the recurrence rate was 3.57%,his blood routine and urine routine were normal.2cases in the animal protein-free diet group had rash recurrence,the recurrence rate was 6.67%,their blood routine and urine routine were normal.The χ~2 test would be used to the recrudescent rate of the common diet group and animal protein-free diet group,there were no significant difference.conclusion1.General food and animal-free protein diet group had no significant effect on the length of hospitalization,resolution of rash,abdominal pain,discharge blood routine NLR and PLR of children with mixed Ig A vasculitis.2.General food is more conducive to the recovery of joint swelling and pain in children with mixed Ig A vasculitis than animal protein-free diet.3.In the short term(within 4 weeks),general food and animal protein-free diet have an impact on the weight growth of children,and general food is more conducive to the weight growth of children with mixed Ig A vasculitis.4.Children with restricted diet are more likely to develop anxiety in both acute or recovery periods.
