| Objective:The purpose of this study was to evaluate the improvement of the overall function of stable angina pectoris by cardiopulmonary exercise test(CPET)after6 months of intensive management of individualized cardiac rehabilitation program. Methods:In this study,The study subjects were human subjects diagnosed with stable angina pectoris at the Affiliated Hospital of Yan’an University from October 2021to December 2022.At present,The study procedure was briefly introduced to the hospitalized patients with stable angina.According to the basic-level guidelines for the cardiac rehabilitation of coronary heart disease(2020),the low-and middle-risk patients were selected, and the intervention plan was informed. The subjects who were willing to undergo rehabilitation treatment were divided into control group and test group according to the random number method.Test group:On the basis of the treatment prescription written by the cardiologist, the overall program of individualized precise exercise for 6months was implemented to strengthen the control;Control group:On the basis of the treatment prescribed by the cardiologist, cardiac rehabilitation education was carried out according to traditional methods. The cardiopulmonary exercise test and psychological scale assessment were reviewed for the first time and 6 months after discharge in the two groups to compare whether there were differences in cardiopulmonary test indexes and psychological scales for the first time and 6 months after discharge. The changes of cardiopulmonary test index and psychological scale were observed for the first time and6 months in the experimental group. All the collected data were statistically analyzed using IBM SPSS 22,and P<0.05 was considered statistically significant.Results:(1)The general data of the two groups were compared, indicating no statistical significance(P>0.05);The cardiopulmonary test index, psychological scale and perception questionnaire were used to compare the conditions of subjects in the test group and the control group at the first admission, indicating no statistical significance(P>0.05).(2)After discharge, the test group underwent cardiopulmonary exercise test again after 6 months of intensive control, and the indexes of the test group and the control group were compared.Peak oxygen uptake, anaerobic threshold oxygen uptake, maximum oxygen pulse, anaerobic threshold METS,METS max,1-minute heart rate recovery and d VO2/d WR in both groups were significantly better than those in the control group.Among them,Cohen’s d effect sizes of peak oxygen uptake, anaerobic threshold oxygen uptake, maximum oxygen pulse, anaerobic threshold METS,METS max and d VO2/d WR were all greater than 0.80,indicating that the test group and the control group had great differences in the corresponding index data. For the rest, bullseye rate, anaerobic threshold heart rate,VE/VCO2 slope,VE/VCO2pre-VATslope and peak RER showed no statistical significance(P>0.05).(3)By comparing cardiopulmonary exercise indexes before and after intervention,it was found that peak oxygen uptake, oxygen uptake, maximum oxygen uptake pulse,METS,METS max,and d VO2/d WR were significantly better in the test group than before intervention. In addition,Cohen’s d effect sizes were all greater than 0.80,indicating a large degree of data difference before and after intervention of the experimental group.(4)The cardiac function of the test group and the control group was evaluated by Weber standard, which indicated that the cardiac function of the test group had obvious improvement after the intensive control intervention.(5)Compared with the control group, the subjects in the test group had better compliance.(6)Compared with those before the intervention, the scores of the psychological scale and the perception questionnaire of the subjects in the test group showed that both of them were improved after the intensive control intervention, and the differences were statistically significant(P<0.05).In the control group compared to before the intervention, the difference before and after perception questionnaire was conducted again 6 months later was statistically significant(P<0.05).Conclusion:For stable angina patients undergoing cardiac rehabilitation for the first time after evaluation, the application of individualized cardiac rehabilitation program intensive control strategy can improve the cardiopulmonary exercise function, improve patients’mental state and quality of life, and improve patients’treatment compliance has a good effect. |
