Correlation Study And Risk Factor Analysis Of Preoperative PLR NLR And Tumor Diameter On CN0 Stage Micropapillary Thyroid Carcinoma

Objective:To investigate the independent risk factors for the clinicopathological characteristics of central zone lymph node metastasis in stage cN0(Papillary Thyroid Microcarcinoma,PTMC).The maximum value of Platelet-to-lymphocyte Ratio(PLR),Neutrophil-to-lymphocyte Ratio,(NLR)and the combined diameter of the three tumors were analyzed as the predictive value of PTMC.Methods:The clinical and pathological data of PTMC patients with cN0 stage(162 cases)admitted to the Affiliated Hospital of Yan’an University from December 2018 to December 2021,Analysis of the central area lymph node metastasis risk factors and independent risk factors application ROC curve to predict preoperative NLR,PLR tumor maximum diameter and the combined index of the central area lymph node metastasis and predicted value.Results:1.Univariate analysis results show that central area lymph node metastasis and nodules rich tumor diameter nodules multifocal NLR,PLR,Lymphocyte/Monocyte Ratio(LMR),TSH(all p<0.05),And with age and gender tumor location combined Hashimoto nodular goiter calcification and whether clear boundary is not upright(all p>0.05)2.Multivariate analysis suggests that tumor diameter NLR,PLR preoperative TSH is an independent risk factor for cN0 PTMC CLNM(all p<0.05)3.Construction of NLR PLR tumor diameter and combined index ROC curve,The optimal threshold value for NLR for predicting PTMC central zone lymph node metastasis is 1.58(sensitivity 94%,Specificity of 42%,Area size under the roc curve 0.653),PLR the optimal cutoff for predicting lymph node metastasis in PTMC at cN0 is 117.20(sensitivity 81%,Specificity of 44%,Area size under the roc curve:0.627)the optimal threshold value for predicting cN0 is 0.65(sensitivity 62%,Speciality 72%,Area size under the ROC curve:0.693)The optimal cut-off value for predicting lymph node metastasis in the central zone of cN0 is 0.24(sensitivity 85%,Speciality 55%,Area size under the ROC curve:0.726)Conclusion:the diameter size of nlrplr tumor and the combined index of the three for judging cN0 PTMC CLNM has certain clinical prediction significance for the choice of preoperative surgical mode for clinicians,Provide the necessary basis,Has some potential value.
