| Objective: To explore the efficacy and prognosis of surgery in the treatment of tuberosity fractures in the elderly under the green channel.Methods: A total of 100 patients with tuberosity fracture of the femur from June2019 to December 2020 were enrolled in the Lower Limb Ward of the Department of Trauma and Orthopedics of Xi’an Red Society Hospital from June 2019 to December2020,and 50 patients with tuberosity fracture of the femur admitted from June 2019 to December 2019 were selected as the routine group for routine treatment.A total of 50 patients with tuberosity fracture of the femur admitted from January 2020 to December2020 were selected as the green channel group,and the green channel process was given.The preoperative general data,intraoperative operation time,intraoperative blood loss,postoperative complications,hip function Harris score,quality of life score and other related indicators of the two groups were compared.SPSS25.00 software was used for analysis,and P<0.05 was statistically significant.Results: All patients in both groups completed the surgery successfully,and patients were followed up for 15 to 24 months [(18.8±6.5)months].There was no statistically significant preoperative general data gender,age,fracture subtype,and combined medical diseases in both groups(all P > 0.05);the preoperative waiting time in the green channel group was lower than that in the conventional group,which was statistically significant(P < 0.001).In the comparison of mortality rates,there was no statistically significant difference between the green channel group and the conventional group in terms of mortality rates at 1 month,3 months and 6 months after surgery(all P>0.05);the mortality rate of patients in the green channel group at 1 year after surgery was significantly lower than that in the conventional group,which was statistically significant(P<0.05);in the comparison of perioperative data,the operating time,hospitalization time and postoperative bed rest time in the green channel group were significantly lower than those in the conventional group(P < 0.05);intraoperative bleeding,postoperative one-day drainage,and occult blood loss in the green channel group were significantly less than those in the conventional group(P<0.001).In comparison of postoperative complications,the incidence of delayed healing in the green channel group was lower than that in the conventional group,which was statistically significant(P=0.044);there were 3 cases of hip pain in the green channel group and 6cases of hip pain in the conventional group,which was statistically significant(P=0.036);there were 2 cases of pressure sores in the green channel group and 5 cases of pressure sores in the conventional group,which was statistically significant(P=0.018);there was1 case of incisional infection in the green channel group and 3 cases of incisional infection in the conventional There were 3 cases of incisional infection in the green channel group and 3 cases in the conventional group,which was statistically significant(P=0.041).There was no statistically significant difference between the two groups in the incidence of postoperative venous thrombosis,incidence of internal fixation loosening,incidence of myocardial infarction,and incidence of pulmonary infection(all P>0.05).When comparing the postoperative hip function Harris scores,the postoperative hip function Harris scores at 3 and 6 months were significantly higher in the green channel group than in the conventional group(P < 0.05).Conclusion: Although the green channel treatment of elderly femoral trochanter fracture cannot affect the short-term mortality rate of patients,it can reduce the mortality rate of patients 1 year after surgery,reduce the occurrence of postoperative complications,and help patients recover rapidly from postoperative hip function,which is in line with the concept of rapid rehabilitation and worthy of wide clinical promotion. |
