| Purpose:To investigate the reliability and clinical value of the Critical-Care Pain Observation Tool-Neuro(CPOT-Neuro)and the Behavioral Pain Scale-Non Intubated(BPS-NI)for clinical pain assessment in patients with disorders of consciousness(DOC).Methods:The CPOT-Neuro and BPS-NI were used to score non-painful stimuli and painful stimuli in 36 patients with DOC in the Department of Rehabilitation Medicine of the First Affiliated Hospital of Nanchang University,and the internal consistency,content validity,structural validity,criterion-related validity of the above scales were analyzed and the best cut-off values were selected.Results:CR values for each item of the CPOT-Neuro ranged from 1.500 to12.728;the correlation coefficients between item score and total score ranged from0.355 to 0.803;the Cronbach’s α was 0.684;the I-CVI and S-CVI were both 1;the factor loadings of each item ranged from 0.598 to 0.909,and criterion-related validity was 0.939,p< 0.001;the AUC value was 0.966,p < 0.001.The Cronbach’s αof the CPOT-Neuro revision which was formed after CPOT-Neuro deleted the item of tearing/facial Flushing was 0.711;the I-CVI and S-CVI were both 1,the factor loadings of each item ranged from 0.597 to 0.858,and criterion-related validity was0.935,p < 0.001;the AUC value was 0.965,p < 0.001;the best cut-off value was 1.CR values for each item of the BPS-NI ranged from 5.382 to 10.776,P<0.001;the correlation coefficients between item score and total score ranged from 0.725 to0.834;the Cronbach’s α was 0.715;I-CVI and S-CVI were both 1;factor loadings for each item ranged from 0.758 to 0.873,and the correlation validity of the validity was 0.890,P < 0.001;the AUC value was 0.966,P < 0.001;the best cut-off value was 4.Conclusion:The CPOT-Neuro revision,which was formed after deletion of item of tearing/facial flushing,as well as BPS-NI,has good reliability and high diagnostic value and can be used as a pain assessment tool for clinical DOC patients. |
