| Objective:Through early plasma exchange treatment of patients with hypertriglyceridemia acute pancreatitis(HTG-AP),early,rapid and effective removal of high levels of TG,evaluate the efficacy of plasma exchange in patients with HTG-AP,and analyze whether the patient prognosis.Methods:Collecting patients diagnosed with HTG-AP incorporated into our hospital from June 31,2017 to July 1,2022,General patient data and laboratory examination indicators were summarized for baseline level comparison,And according to whether the patient underwent plasma exchange therapy is divided into plasma exchange group and conventional therapy group,Blood triglycerides(TG),cholesterol(TC),blood pancreatic enzyme,serum inflammatory index,disease severity score and prognosis index were collected from the two groups for comparative analysis,To evaluate the efficacy of plasma exchange compared to conventional therapy,Analyanalyzed the disease progression and prognosis,To evaluate the clinical value of plasmapheresis therapy in patients with HTG-AP.Result:There were no significant differences in TG,TC,amylase,lipase,C reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor,acute physiology and chronic health score(APACHE Ⅱ score),and SOFA score before treatment(p>0.05);the groups were statistically different(p<0.05);after comparing post-treatment indicators,there was no significant difference in plasma exchange group except for blood amylase and blood leukocyte index and the conventional treatment group.The other indicators were significantly lower compared with the conventional treatment group(p<0.05);there was no significant difference in the total length of stay and ICU stay between the two groups(p>0.05),and the hospitalization cost in the plasma exchange group was higher than that in the conventional treatment group(p<0.05).Conclusion:Compared with conventional lipid-lowering methods,early plasmapheresis therapy could significantly improve SOFA score,APACHE Ⅱ score,blood,TG,TC,amylase,lipase and CRP,IL-6,and TNF-α in patients with HTG-AP,but did not significantly improve the prognosis of patients. |
