Objective:To compare the changes of erythrocyte sedimentation rate,C reactive protein,ASIA injury grade and Frankel grade in patients with spinal tuberculosis by group study on the length of preoperative anti-tuberculosis time in patients with spinal tuberculosis,to analyze the effect of preoperative anti-tuberculosis treatment time on the prognosis of patients with spinal tuberculosis,and to explore the timing of surgical intervention.Methods:From January 2019 to September 2021,49 patients diagnosed with spinal tuberculosis and received surgical treatment in our department,consisting of 27 male cases and 22 female patients;Age 47~73 years old,14 cases of thoracic tuberculosis,25 cases of lumbar tuberculosis,10 cases of thoracic spine combined with lumbar tuberculosis;According to the time of anti-tuberculosis reception before surgery,patients in group A received conventional H-R-Z-E anti-tuberculous therapy for 2~4weeks(average 3.3 weeks)after admission,and patients in group B received anti-tuberculosis treatment for 1 ~ 2 weeks(average 1.5 weeks)after admission.The changes of preoperative and postoperative pain VAS,ESR,CRP,Frankel grade,and ASIA injury grade were recorded in the two groups.SPSS22.0 software was used for data processing and analysis;t test was used for intra-group and inter-group comparison;Chi-square test was used for counting data;P < 0.05 was considered statistically significant.Results:All 49 patients were followed up,the length of anti-tuberculosis treatment time in the group A and group B was statistically significant on preoperative erythrocyte sedimentation rate(ESR)and C reactive protein(P<0.05),the difference in operation time and surgical blood loss was not statistically significant(P>0.05),there was no postoperative spread of tuberculin and recurrence of tuberculosis in both groups,the comprehensive pain score and nerve damage score of both groups were significantly improved with time,and the scores of the two groups in the same time period were not statistically significant(P>0.05).Conclusion:After receiving 1~2 weeks and 2~4 weeks of anti-tuberculosis therapy before surgery,there is no significant difference in the prognosis between the two patients and satisfactory efficacy is obtained,indicating that after admission,exclusion related surgical contraindications can be treated positive as long as the erythrocyte sedimentation rate(ESR)and Creactive protein(CRP)decrease or in a downward trend. |
