| Objectives:Through systematic evaluation and meta-analysis,this study evaluated the effects of high torque implants on implant failure rate and implant edge bone loss compared with conventional torque implants,so as to evaluate the clinical effect of high torque implants.Method:Two independent assessors searched the documents from the establishment of English database(PubMed,web of science,Cochrane Library)and Chinese database(China knowledge network,Wanfang medical network and VIP network)to October 2022 through retrieval strategy,manual retrieval,screened and extracted the searched documents according to the established inclusion criteria and exclusion criteria.For the selected literatures,the Cochrane bias risk assessment tool and NOS scale were used to evaluate the literature bias and quality.If there are differences,they should be settled through consultation with the third evaluator.Then use revman5 4 and stata17 0 software used relative risk ratio(RR)and mean difference(MD)as effect analysis statistics for meta-analysis of the main outcome indicators implant failure rate and marginal bone loss in the data extracted from the included literature.Result:By searching,this study eventually included 11 research articles,a total of 835 patients,and finally counted 1400 implants,including 870 high torque implants and 530 conventional torque implants.The results of meta-analysis showed that there was no significant difference in implant failure rate between high torque implant and conventional torque implant(RR=1.25,95%CI:[0.69,2.29],P=0.46);In the subgroup analysis of implant failure rate,there was no significant difference in implant failure rate between high torque implant and conventional torque implant with follow-up time<1 year(RR=0.30,95%CI:[0.02,3.60],P=0.34).There was significant difference in implant failure rate between high torque implant and conventional torque implant with follow-up time of 1-3 years(RR=3.23,95%CI:[1.31,7.94],P=0.01).As for the amount of marginal bone loss,there was significant difference between high torque implant and conventional torque implant(RR=0.16,95%CI:[0.03,0.30],P=0.02);There was no significant difference in the amount of marginal bone loss between high torque implants and conventional torque implants with a follow-up time of<1 year(RR=0.03,95%CI:[-0.01,0.06],P=0.15),while there was a significant difference in the amount of marginal bone loss between high torque implants and conventional torque implants with a follow-up time of 1-3 years(RR=0.38,95%CI:[0.28,0.49],P<0.05).Conclusion:1.When the follow-up time is less than 1 year,there is a similar implant failure rate between high torque implant and conventional torque implant,and there is no significant difference in bone resorption around the implant.2.During the follow-up period of 1-3 years,compared with conventional torque implants,high torque implants have a higher failure rate,and high torque implants also have more obvious marginal bone resorption. |
