Study On Status And Influencing Factors Of Knowledge,Attitude And Practice On The Breast Cancer Rehabilitation Among Nurses In Department Of Breast Cancer

Objectives:1.To develop a knowledge-attitude-practice(KAP)Questionnaire on the Breast Cancer Rehabilitation for nurses in the Department of Breast Cancer.2.To investigate the KAP status on the breast cancer rehabilitation of nurses in the Department of Breast Cancer and to analyze influencing factors.Methods:1.Based on the theory of KAP,the item pool of the Questionnaire on the Breast Cancer Rehabilitation for Nurses in the Department of Breast Cancer was formed through group discussion in combination with relevant guidelines and literatures.Delphi method was adopted to invite experts in the field of breast cancer rehabilitation to conduct two rounds of consultation and test the reliability and validity of the questionnaire,forming the final draft of the KAP Questionnaire on the Breast Cancer Rehabilitation for nurses in the Department of Breast Cancer.2.Questionnaire survey: stratified random sampling was used.Firstly,8 cities were randomly selected in the east,south,west and north of Hunan Province,and 2 cities were randomly selected in each part.1tertiary hospital and 1 secondary hospital were randomly selected in each city,a total of 16 hospitals were selected,and all nurses in the Department of Breast Cancer of those 16 hospitals were selected as research objects.A self-designed questionnaire was used to investigate nurses in the Department of Breast Cancer of the above 16 hospitals.3.SPSS 26.0 software was used for statistical analysis of the survey data,meanwhile the mean,standard deviation and constituent ratio were used for statistical description.T-test and analysis of variance(ANOVA)were used for univariate analysis,and multiple linear regression analysis was used for multivariate analysis.Results:1.Results of Delphi expert consultation:(1)the response rates of the two rounds of expert consultation were 94.44% and 100%,the expert authority coefficient was 0.959.In the first round,the average importance scores of the items were 4.24～4.94,and coefficients of variation of the items were 0.05～0.23.In the second round,the average importance scores of the items were 4.35～5.00,and the coefficients of variation were0.00～0.20.(2)After expert consultation,56 items were finally determined,including 22 knowledge items,12 attitude items and 22 practice items.2.Reliability and validity test results:(1)the Cronbach’s α coefficient of the questionnaire was 0.949.(2)The S-CVI/Ave of the questionnaire was 0.98.3.The score of knowledge dimension on breast cancer rehabilitation of nurses in the Department of Breast Cancer was(11.24±2.86)with a scoring rate of 51.09%.The score of attitude dimension was(54.08±9.87),with a scoring rate of 90.13%;The score of practice dimension was(87.85±17.87),with a scoring rate of 79.86%.4.Univariate analysis showed that the main influencing factors of nurses’ knowledge scores on the breast cancer rehabilitation were professional titles(F=8.596,P< 0.001),working years in the Department of Breast Cancer(F= 4.749,P=0.003),first education(F=4.604,P<0.011),highest education(F=10.808,P<0.001),hospital grade(t=4.902,P<0.001),hospital type(F=12.462,P< 0.001)and whether they are specialist nurses of oncology(t=2.307,P= 0.022).The main influencing factors of nurses’ attitudes towards the breast cancer rehabilitation were first education(F=3.822,P=0.023),the frequency of nurses’ participation in professional learning related to breast cancer rehabilitation in the department(t=-2.046,P=0.042),whether they are specialist nurses of oncology(t=2.032,P=0.043)and whether there was a breast cancer rehabilitation process in their department(t=4.170,P<0.001).The main influencing factors of nurses’ practice scores on the breast cancer rehabilitation were hospital type(F=5.889,P=0.003),the frequency of nurses’ participation in professional learning related to breast cancer rehabilitation in the department(t=-4.519,P< 0.001),whether they participated in the training class of breast cancer rehabilitation(t=3.769,P< 0.001)and whether there was a breast cancer rehabilitation process in their department(t=5.019,P< 0.001).5.The results of multiple linear regression analysis showed that the main influencing factors of knowledge scores on the breast cancer rehabilitation of nurses were highest education(t=2.809,P=0.005),hospital type(t=-3.197,P=0.002),working years of nurses in the Department of Breast Cancer(t=2.753,P=0.006)and first education(t=2.049,P= 0.041).The main influencing factor of scores of nurses’ attitude towards breast cancer rehabilitation was whether there was a breast cancer rehabilitation process in their department(t=4.170,P<0.001).The main influencing factors of practice scores on the breast cancer rehabilitation of nurses were whether there was a breast cancer rehabilitation process in their department(t=3.883,P< 0.001),hospital type(t=-3.362,P= 0.001),the frequency of nurses’ participation in professional learning related to breast cancer rehabilitation in the department(t=2.962,P=0.003)and whether they participated in the training class of breast cancer rehabilitation(t=-2.204,P= 0.028).Conclusion:1.The questionnaire of KAP on the breast cancer rehabilitation for nurses in the Department of Breast Cancer has good reliability and validity,which can be used to investigate the status of knowledge,attitude and practice on the breast cancer rehabilitation for nurses.2.Nurses in the Department of Breast Cancer have a positive attitude towards breast cancer rehabilitation,but their knowledge and practice need to be improved.3.The main influencing factors of nurses’ knowledge scores on the breast cancer rehabilitation were highest education,hospital type,the working years of nurses in the Department of Breast Cancer and first education.The main influencing factor of nurses’ attitude towards the breast cancer rehabilitation was whether there was a breast cancer rehabilitation process in their department.The main influencing factors of nurses’ practice scores on the breast cancer rehabilitation were whether there was a breast cancer rehabilitation process in their department,hospital type,the frequency of nurses’ participation in professional learning related to breast cancer rehabilitation in the department and whether they participated in the training class of breast cancer rehabilitation.It is suggested that the breast cancer rehabilitation training needs to be strengthened for nurses with low education and short working years in the Department of Breast Cancer,and those who work in other non-tumor professional hospitals,as well as those who have not participated in the professional training courses on the breast cancer rehabilitation.The optimization and management of breast cancer rehabilitation process in the department should be strengthened.
