IL-8 And RANKL In Peri-Microscrew Implant Inflammation Correlation Analysis Between Expression Level And Clinical Periodontal Index

Objective:In this study,peripheral fluid of microscrew implants was collected at different cycles and enzym-linked immunosorbent assay(ELISA)was used to detect interleukin-8.IL-8)and the expression level of Receptor or Activator of NF-κB Ligand(RANKL)to initially explore the pathologic mechanism of microscrew peri-implant inflammation.To explore whether IL-8 and RANKL can be used as indicators to reflect the health status of the tissue around microscrews.Combined with the clinical periodontal index and relevant data,the relevant factors affecting the stability of microscrew were analyzed,which provided data reference for the early evaluation of the tissue health around the microscrew implant by the clinical periodontal index and the thinking direction for future treatment plan selection.Method:(1)A total of 48 microscrews were collected from 24 patients who were admitted to the Orthodontics Department of the Affiliated Hospital of Youjiang Medical College for Nationalities from August 2021 to November 2022 and required microscrew implantation due to treatment plan.The average age of the subjects in this study ranged from18 to 28 years old,among whom 13 were male and 11 were female.According to the health conditions of the tissues around the implant nails,they were divided into stable group(n=28pins)and inflammatory group(n=20 pins).The fluid around the microscrew was collected and weighed at 1,4 and 12 weeks after microscrew implantation,and the concentration expressions of IL-8 and RANKL were detected by ELISA,and the changes of periodontal indexes were recorded at different periods.SPSS 26.0 software was used for statistical analysis of the data,and the quality expression of fluid around microscrews and the concentration expression of IL-8 and RANKL were compared by two-factor repeated measurement ANOVA.Periodontal indexes between different groups were compared by Mann-Whitney U rank sum test,which was represented by M(Q25,Q75).Multiple linear regression analysis was used to analyze the correlation between clinical periodontal index affecting the concentration expression of IL-8 and RANKL.The correlation between IL-8 and RANKL concentration was compared by spearman correlation analysis.For all statistical results,P < 0.05 was statistically significant.Results:(1)The quality of fluid around microscrews was compared: in the stable group,the peak value was reached at week 1,and the difference was statistically significant compared with week 4 and 12(P < 0.05).There was no significant difference between 4weeks and 12 weeks(P > 0.05).The expression of inflammation group increased with the passage of the cycle,and the difference was statistically significant(P < 0.05).The difference between stable group and inflammatory group was statistically significant(P < 0.05).(2)Comparison of IL-8 concentration difference: The concentration level of the stable group reached the peak at week 1,and the difference was statistically significant compared with week 4 and 12(P < 0.05).There was no significant difference between 4 weeks and 12 weeks(P > 0.05).The level of IL-8 in the inflammation group increased with the passage of the cycle,and the difference was statistically significant(P < 0.05).The difference between stable group and inflammatory group was statistically significant(P < 0.05).(3)Comparison of RANKL concentration differences: The concentration of stable group showed a decreasing trend with the passage of the cycle,and the difference was statistically significant(P < 0.05);The level of RANKL in inflammation group increased with the passage of the cycle,and the difference was statistically significant(P < 0.05).There was significant difference between stable group and inflammatory group(P < 0.05).(4)Comparison of periodontal indexes between groups: The comparison of PD(probing depth,PD)and mBI(modification sulcus bleeding index,mBI)groups had statistical significance in the three cycles(P < 0.05).There was no significant difference in m PLI(modification plaque index,m PLI)at week 1(P > 0.05).(5)Analysis of the influence of IL-8 and RANKL concentration levels and periodontal related indicators: PD and mBI were important factors affecting the expression levels of IL-8 and RANKL(P < 0.05).(6)There was a positive correlation between the concentration of IL-8and RANKL in the surrounding fluid(P<0.05).Conclusions:(1)The high expression levels of fluid quality,IL-8 and RANKL around microscrew may indicate the occurrence or progression of microscrew peri-implant inflammation.Considering these indexes may be important factors affecting the stability evaluation of microscrew implants.(2)By analyzing the concentration expression levels of IL-8 and RANKL in peri-implant inflammation of microscrews,the two factors could be considered as indicators to evaluate the periodontal tissue health status of microscrews,and the stability of microscrews could be improved by regulating related factors in the future.(3)In periodontal index,PD and mBI are the main factors affecting the expression of IL-8 and RANKL concentration.Considering PD and mBI index can provide relevant data reference for guiding the early evaluation of the health status of the tissues around microscrews and provide a new direction of thinking for the prevention and treatment of microscrew periinflammation in the future.
