| Objective: In this study,silicone dressings were used as experimental drugs to explore the efficacy and safety of silicone dressings in the treatment of keloid by surgery combined with 90 Sr radiotherapy,such as the analysis of the effect of silicone dressings in reducing the side effects of 90 Sr radiotherapy and reducing the postoperative recurrence rate of keloids,so as to provide reference for clinical application of silicone dressings in the treatment of keloids.Methods: This subject adopted the method of randomized controlled study.The preliminary study subjects were from September 2020 to June 2022 to the Department of wound repair and plastic surgery of the affiliated Hospital of Youjiang Medical College for nationalities.The outpatients diagnosed as keloid were treated with surgery and postoperative combined with 90 Sr radiotherapy.According to different treatment methods,the patients were divided into two groups: postoperative 90 Sr irradiation group(n = 75)and postoperative 90 Sr combined with silicone dressing group(n = 75).The experimental group was sprayed with silicone dressing immediately after postoperative 90 Sr radiotherapy and the operation was basically healed,3 times a day,while the control group was not treated or only scrubbed with normal saline.The overall efficacy,local skin pruritus,pain and pigmentation,Vancouver scar scale(Vancouverscarscale,VSS)score and recurrence rate were compared between the two groups after 6 months treatment.Results: A total of 143 patients completed this study,including 71 cases in the control group(75 scars)and 72 cases in the experimental group(79 scars).There was no significant difference in sex,age,scar site and preoperative Vancouver scar scale scores between the control group and the experimental group(P > 0.05).The study is feasible.In terms of therapeutic effect,there was no significant difference in local skin pain,itching and vascular distribution between the control group and the experimental group(P> 0.05).The results of the experimental group were significantly better than those of the control group in color,thickness,softness,pigmentation and postoperative VSS score(P<0.05).The effective rate of the experimental group was 92%.The effective rate of the control group was 81%,and the treatment effect was more significant by statistical test(P<0.05).In terms of recurrence rate,the recurrence rate of the experimental group(8%)was lower than that of the control group(19%),and the statistical difference was significant(P<0.05).During the observation period,there were no local or systemic serious adverse reactions in both the experimental group and the control group.Conclusion:(1)Silicone dressing on the basis of surgery combined with 90 Sr radiotherapy can not improve local pruritus,pain and vascular distribution,but can significantly improve scar pigmentation,thickness and softness,and down-regulate the overall VSS score.(2)This study further proves that postoperative 90 Sr radiotherapy for keloid is a safe and effective method.On this basis,the combined use of silicone dressings has positive significance for the prevention of scar recurrence. |
