Effect Of Huazhuo Qiangjing Decoction On Sperm Reactive Oxygen And Mitochondrial Membrane Potential In The Treatment Of Idiopathic Asthenospermia (Kidney Deficiency And Stasis And Toxicity Type)

Objective1.Through the observation of the changes of sperm active oxygen species,sperm mitochondrial membrane potential,percentage of forward movement,total activity and traditional Chinese medicine symptoms/syndromes in patients with idiopathic asthenospermia(kidney deficiency and blood stasis and toxin type)after cure,the mechanism of Huazhuo Qiangjing decoction in treating idiopathic asthenospermia(kidney deficiency and blood stasis and toxin type)was preliminarily explored;2.To probe into the availability and safety of Huazhuo Qiangjing decoction in the cure of sufferers with Idiopathic sperm motility((type of deficiency of the kidney and blood stasis and toxin).MethodThe patients with idiopathic asthenospermia who visited from January 2022 to October 2022 were collected from the outpatient department of the Reproductive Center of Anhui Provincial Hospital of Traditional Chinese Medicine,and 60 patients with idiopathic asthenospermia(kidney deficiency,stasis and toxin type)who met the diagnostic criteria and inclusion criteria of this study were selected and randomly divided into two groups.Among them,30 patients in the control group were treated with L-carnitine oral solution for 3 months.30 patients in the treatment group were treated for 3 months with Huazhuo Qiangjing Decoction.Observe,analyze and summarize the changes of normal sperm percentage of mitochondrial membrane potential,percentage of forward moving sperm,percentage of fast straight forward moving sperm,sperm reactive oxygen species level,and traditional Chinese medicine syndrome score of the two groups of patients.ResultDuring the remedy,2 cases in the control group voluntarily withdrew,and 1 case in the treatment group fell off due to poor compliance.Finally,28 cases in the control group and 29 cases in the treatment group were included in the statistics;The baseline data(age,course of disease,BMI)of the two groups were evenly distributed(P>0.05).The specific results of key indicators are as follows:1.1 Comparison of forward motile sperm(PR)percentage between the Huazhuo Qiangjing decoction group and the Levocarnitine group.Before cure,the PR percentage of patients in the two groups was evenly distributed(P>0.05);After treatment,the percentage of PR in both groups was significantly higher than that before(P<0.01),and compared with the control group,the treatment group had better effect(P<0.05).1.2 Comparison of total sperm activity(PR+NP)between the therapy group and matched group.Before treatment,PR+NP was evenly distributed in the two groups(P>0.05);After treatment,the PR+NP of patients in both groups was clearly higher than that before(P<0.01).Compared with the Levocarnitine group,the therapeutic effect in the Huazhuo Qiangjing decoction group was better(P<0.01).1.3 Comparison of percentage of rapid forward motility sperm(QPR)between the Huazhuo Qiangjing decoction group and the Levocarnitine group of patientsBefore remedy,the distribution of QPR in the two groups was uniform(P>0.05);After treatment,the QPR of patients in both groups was clearly higher than that before(P<0.01).Compared with Levocarnitine group,Huazhuo Qiangjing decoction group had a better effect(P<0.01).1.4 Comparison of ROS levels between Huazhuo Qiangjing decoction group and Levocarnitine groupThere was no significant importance in ROS level of sperm cells between Huazhuo Qiangjing decoction group and Levocarnitine group before remedy(P>0.05);The ROS level of sperm cells in the two groups after remedy was obvious topper than cure before(P<0.01).The remedy group was better(P<0.01).1.5 Comparison of normal sperm percentage of MMP between two groups of patientsThere was no statistical sense in the percentage of normal sperm of MMP between Huazhuo Qiangjing decoction group and Levocarnitine group before remedy(P>0.05);The percentage of normal sperm of MMP in the two groups after remedy was clearly topper than cure before(P<0.01).The Huazhuo Qiangjing decoction group was better(P<0.01).1.6 Comparison between ROS and PR,PR+NP in two groups of patientsBefore remedy,ROS was obviously positive interrelated with PR and PR+NP in both groups(P<0.01).After positive correlation between the transformation of MMP value and the transformation of PR and PR+NP values in both groups(P<0.01).1.8 Comparison of traditional Chinese medicine efficacy scores between Huazhuo Qiangjing decoction group and Levocarnitine group1.8.1 Traditional Chinese medicine syndrome scoreBefore remedy,the TCM syndrome scores of the two groups were evenly distributed(P>0.05);After remedy,the TCM syndrome score of the two groups decreased compared with that before(P<0.01),and the therapeutic effect of the treatment group was better(P<0.01).1.8.2 Evaluation of curative effect of traditional Chinese medicineThe total effective rate of TCM treatment in the treatment group and the control group was 93.10% and 67.86% respectively,and the treatment group was better(P<0.05).1.9 Clinical efficacy resultsIn general,the active rate of the matched group was 71.4%,and the total active rate of the remedy group was 89.7%.The remedy effect was stronger than that the matched group,and the transformation was statistically sense(P<0.05).1.10 Safety observationThere were no obvious complaints of discomfort during the study in both groups,and there were no reported abnormalities in body temperature,heart rate,liver and kidney function and other indicators after study.Conclusion:1.Huazhuo Qiangjing decoction may improve sperm vitality by reducing the production of ROS and maintaining the normal mitochondrial membrane potential level of sperm;2.Huazhuo Qiangjing Decoction can im PRove sperm vitality,im PRove TCM related symptoms of sufferers with type of deficiency of the kidney and blood stasis and toxin,and has significant clinical effect and high safety in the remedy of idiopathic asthenospermia.
