| Objective: To understand the current status of empowerment ability,self-perceived burden and quality of life in continuous ambulatory peritoneal dialysis patients,and to explore the influencing factors of the three,and to further verify whether self-perceived burden can play a mediating role between empowerment ability and quality of life on the basis of analyzing the correlation between empowerment ability,self-perceived burden and quality of life.Methods: From February 2022 to August 2022,a total of 240 continuous ambulatory peritoneal dialysis inpatients in two wards of the Department of Nephrology of the First Affiliated Hospital of Anhui University of Chinese Medicine were selected by convenient sampling and investigated with the self-designed general information questionnaire,Peritoneal Dialysis Empowerment Scale,Self-perceived Burden Scale and Kidney Disease Quality of Life Questionnaire.SPSS 23.0 software was used for statistical analysis of data,and numerical variable data were expressed as (?)±s.Categorical variable data were expressed as frequency and constituent ratio.The t test and analysis of variance were used to compare the general data between single factor groups.Pearson correlation analysis was used to analyze the correlation between variables.Multiple linear regression was used to analyze the multivariate influence of variables.AMOS 24.0 structural equation model was used to analyze the mediating effect of self-perceived burden,and Bootstrap method was used to further verify.Results: 1.The total score of empowerment ability of 240 continuous ambulatory peritoneal dialysis inpatients was(22.83±4.16);The total score of self-perceived burden was(33.04±6.12);The total score of quality of life was(76.65±7.01).2.The results of Pearson correlation analysis showed that the total score of empowerment ability was significantly negatively correlated with the total score of self-perceived burden(r=-0.558,P<0.01);The total score of empowerment ability was significantly positively correlated with the total score of quality of life(r=0.574,P<0.01);The total score of self-perceived burden was negatively correlated with the total score of quality of life(r=-0.432,P<0.01).3.Multiple linear regression analysis showed that Age,marital status,education level,main mode of residence,primary disease,dialysis age,body mass index,major complications were the influencing factors of empowerment ability of continuous ambulatory peritoneal dialysis patients(P<0.05),which explained 50.8% of the total variation;Age,marital status,status of employment,main caregiver and main complications were the influencing factors of body burden in continuous ambulatory peritoneal dialysis patients(P<0.05),explaining 17.20% of the total variation;Gender,marital status,main mode of residence and main caregivers were the influencing factors of emotional burden in continuous ambulatory peritoneal dialysis patients(P<0.05),explaining 16.80% of the total variation.Age,place of residence,family monthly income,main mode of residence,main caregivers and primary diseases,body mass index were the influencing factors of the economic burden of continuous ambulatory peritoneal dialysis patients(P<0.05),explaining37.00% of the total variation.Age,marital status,status of employment,family monthly income,main mode of residence,main caregiver,primary disease,dialysis age(months),major complications,empowerment ability and self-perceived burden were the influencing factors of the quality of life of continuous ambulatory peritoneal dialysis patients(P<0.05),explaining 54.80% of the total variation.4.The construction of mediation model found that self-perceived burden could mediate the relationship between mediation ability and quality of life in continuous ambulatory peritoneal dialysis patients,and the fitting index of the model was: χ~2/DF=3.087,GFI=0.914,RMSEA=0.078,CFI=0.951,TLI=0.936,NFI=0.918.The mediating effect of self-perceived burden on the influence of empowerment ability on quality of life was0.207,accounting for 42.33% of the total effect.Conclusion: 1.The total score of empowerment ability of continuous ambulatory peritoneal dialysis patients was slightly lower than the neutral option score.2.There are 69.58% patients had moderate and severe self-perceived burden.It is suggested to pay attention to such high-risk groups and give positive and effective intervention to reduce the burden of patients.3.The total score of continuous ambulatory peritoneal dialysis patients burden of kidney disease and physical health dimension of SF-12 were low.4.Self-perceived burden played a partial mediating role between empowerment ability and quality of life,suggesting that clinical medical staff can take targeted intervention measures to improve the quality of life according to the type of patients. |
