ObjectiveThe purpose of this study is to explore the therapeutic effect of Compound Sanqi Granules on atherosclerosis of"kidney deficiency and blood stasis type",and explore its anti-atherosclerotic mechanism,so as to provide a reasonable reference for clinical application.Methods1)Animal experiment1.Select 48 healthy male Apo E-/-mice and randomly divide them into blank control group,model group,Lipitor group and low,medium and high dose groups of Compound Sanqi Granules;2.The blank control group was fed with ordinary feed,and the other groups were fed with high-fat feed.After 4 weeks of feeding,the model was completed and the drug was continuously administered for 28 days;3.After the experiment,observe the condition of aortic plaque,blood lipid level and serum inflammatory marker level(TNF)of mice in each group under the electron microscope-α、IL-1β、IL-4 and IL-10),PI3K,Akt,m TOR signal pathway and phosphorylation related protein expression level;4.SPSS 22.0 and Graphpad 6.0 and other software were used to statistically analyze the data and make charts.2)Clinical trial1.According to the diagnostic criteria of atherosclerosis and the quantitative criteria of TCM syndrome score required by the literature,56 AS patients who met the inclusion criteria in the geriatric cardiology department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from June 2021 to November2022 were selected as the subjects of this study,and were randomly divided equally.2.The control group was treated with Lipitor,and the treatment group was treated with Compound Sanqi Granules on the basis of the former,with a course of 12 weeks.3.After the end of the experiment,the changes of TCM syndrome score,carotid plaque,blood lipid,serum inflammatory markers(CRP,ICAM-1 and IL-6),oxidative stress response(MDA,SOD and ox-LDL)were recorded before and after treatment in the two groups.4.SPSS 22.0 and Graphpad 6.0 and other software were used to statistically analyze the data and make charts.Results1)Animal test part1.Compared with the blank control group and the model group,the size of aortic plaque,foam cells and lipid deposition in the plaque of mice in the Compound Sanqi Granules group and Lipitor group were improved to varying degrees,and the effect of the high dose of Compound Sanqi Granules group was better than that of Lipitor group;2.Compared with the model group,the blood lipid level of Lipitor group and Compound Sanqi Granule group decreased to different degrees(p<0.01,p<0.05).Compared with Lipitor group,the blood lipid level of Compound Sanqi Granule group mice decreased more significantly(p<0.01,p<0.05);3.Compared with the blank control group,the content of IL-4 and IL-10 in the model group decreased significantly(p<0.01,p<0.05),TNF-α、IL-1βThe content was significantly increased(p<0.01,p<0.05).Compared with the model group,the levels of IL-4,IL-10 and TNF were significantly increased(p<0.01,p<0.05)in Lipitor group and Compound Sanqi Granules in each dose group-α、IL-1βThe content decreased significantly(p<0.01,p<0.05);Among them,the effect of the high dose group of Compound Sanqi Granules was the most significant,and its effect was better than that of Lipitor group;4.Compared with the blank control group,the expression of p-PI3K,P-Akt and p-m TOR in the aorta of the model group mice was significantly decreased.The expression of PI3K,Akt and m TOR in mice of Compound Sanqi Granules group and Lipitor group were significantly increased,among which,the expression of Compound Sanqi Granules was significantly higher,The high dose group was the most obvious(p<0.01).2)Clinical trial1.After the test,the total effective rate of the treatment group was 89.29%,significantly higher than that of the control group 78.57%;2.After the end of the test,the TCM syndrome score level of the two groups was significantly lower(p<0.05),and the lower level of the treatment group was better than that of the control group(p<0.05);3.After the test,the IMT level of carotid artery in both groups was significantly lower(p<0.05),and the lower level in the treatment group was better than that in the control group(p<0.05);4.After the end of the test,the blood lipid levels(TG,TC,LDL-C)of the two groups were significantly lower(p<0.05),and the HDL-C levels were significantly higher(p<0.05),of which the therapeutic effect of the treatment group was better than that of the control group(p<0.05);5.After the end of the trial,the levels of MDA and ox-LDL in the two groups were significantly decreased(p<0.05),and the levels of SOD were significantly increased(p<0.05).The therapeutic effect of the treatment group was better than that of the control group(p<0.05);6.After the end of the trial,the ICAM-1,CRP and IL-6 levels of patients in the two groups were significantly lower on average(p<0.05),and the curative effect level in the treatment group was better than that in the control group(p<0.05).Conclusion1.Compound Sanqi Granule has definite curative effect on atherosclerosis of kidney deficiency and blood stasis type;2.Compound Sanqi Granule can improve the plaque level of Apo E-/-mice and subjects,and its mechanism may be related to the activation of inflammatory immune response and related signal pathways;3.The curative effect of compound Sanqi granules combined with western medicine on atherosclerosis is better than that of single drug treatment.
