Objective: To observe the clinical efficacy of Buyang Huanwu decoction combined with unilateral biportal endoscopy in the treatment of lumbar disc herniation,verify the efficacy of traditional Chinese medicine Buyang Huanwu decoction,and provide clinical reference for the treatment of lumbar disc herniation during perioperative period.Methods: Sixty patients diagnosed with lumbar disc herniation and TCM syndrome differentiation of Qi deficiency and blood stasis admitted to the second Department of Bone Injury of our hospital from June 2021 to June 2022 were selected,according to the method of random Numbers are divided into the treatment group and control group,30 cases each.Treatment group using oral Chinese medicine Buyang Huanwu decoction with unilateral biportal endoscopy therapy,4 weeks after continuous use of traditional Chinese medicine;Control group adopts single dual channel endoscopic conventional treatment.Respectively in the preoperative and postoperative 2 weeks,postoperative 1 month,3 months after operation respectively to observe VAS pain score,two groups of patients JOA low back pain score,dysfunction index ODI score,for the first three month after using modified Macnab curative effect evaluation.Results:All 60 patients completed the study.1.General situation and preoperative observation indicatorsThere were no significant differences in gender,age,course of disease and surgical segment between the two groups(P>0.05),indicating that the data of the two groups were comparable.Two groups of patients in the preoperative VAS score,JOA score,there is no statistical difference of ODI score comparison(P > 0.05),comparable.2.Visual pain analogue scale(VAS)Two groups of patients after 2 weeks,1 month,3 months of VAS scores were compared with preoperative reduced,statistically significant(P < 0.05),and preoperative were gradually reduced.Treatment group after 2 weeks,1 month of VAS score down more significant compared with control group,there are statistical differences(P < 0.05),the treatment group after 3 months of VAS score down,compared with the control group no statistical difference(P > 0.05).3.Japanese Orthopaedic Association Scores(JOA)Two groups of patients after 2 weeks,1 month,3 months JOA scores were compared with preoperative,statistically significant(P < 0.05),and the preoperative are continuing to rise.Treatment group after 2 weeks,1 month,3 months JOA score more significantly than control group increased,with statistical difference(P < 0.05).4.The Oswestry Disability Index(ODI)Two groups of patients after 2 weeks,1 month,3 months of ODI score than preoperative reduced,statistically significant(P < 0.05),and the preoperative continues to reduce.Treatment group after 2 weeks,1 month,3 months more significantly than control group to reduce the ODI score,statistically significant(P < 0.05).5.Modified Mac Nab CriteriaThe treatment group and control group patients after 3 months of total was 93.3%,86.7% respectively,the effect of treatment group is better than the control group,comparison was statistically significant difference(P < 0.05).6.Safety evaluationFollow-up process,two groups of patients with no other obvious adverse drug reaction occurred,show that the safe treatment.Conclusion:Buyang Huanwu Decoction combined with unilateral biportal endoscopy spine surgery for the treatment of lumbar disc herniation has significant efficacy in relieving pain,improving quality of daily life,restoring lumbar function and clinical symptoms of patients,and its therapeutic effect is significantly superior to that of single unilateral biportal endoscopy spine surgery,indicating that the combined traditional Chinese and western medicine is effective in the treatment of LDH,and worthy of clinical application and promotion.
