Objective:To investigate the correlation and predictive value of serum cyclophilin D(CypD)and Ma R1 levels with type 2 diabetes mellitus(T2DM)complicated with metabolic-dysfunction-associated fatty liver disease(MAFLD).Methods:A total of 147 patients with type 2 diabetes who were hospitalized in the Department of Endocrinology,Affiliated Hospital of Chengde Medical College from July 2021 to June 2022 were collected.According to the results of abdominal ultrasound,the subjects were divided into 59 patients with type2 diabetes mellitus(T2DM group)and 88 patients with type 2 diabetes mellitus complicated with MAFLD(T2DM + MAFLD group).Collected the general data and clinical biochemical indexes,and calculate the triglyceride glucose index(Ty G index)= In(TG(mg / dl)× FBG(mg / dl)/ 2).Serum CypD and Ma R1 levels were detected by enzyme-linked immunosorbent assay.To analyze the differences of serum Ma R1 and CypD levels between the two groups and their correlation with glucose and lipid metabolism indexes,and to explore the possible mechanism in the occurrence and development of T2 DM combined with MAFLD.Results:1.Comparison of general clinical information.in the T2 DM + MAFLD group,BMI,WC,WHt R and DBP were higher than the T2 DM group(all P < 0.05).There were no statistically significant differences between the two groups in terms of gender,age,use of lipid-lowering drugs or not,or SBP(all P > 0.05).2.Comparison of clinical biochemical indices between the two groups.Compared with the T2 DM group,there were significantly higher levels of ALT,AST,GGT,UA,TG,FBG,Ty G indices(all P < 0.05)and lower HDL-C levels(all P < 0.05)in the T2 DM + MAFLD group.There was no statistically significant difference between the two groups in SCr,TC,LDL-C and Hb A1c(P > 0.05).3.Comparison of serum Ma R1 and CypD levels in the two groups.Serum CypD levels in the T2DM+MAFLD group were significantly higher than the T2 DM group,while serum Ma R1 levels were significantly lower,with statistically significant differences(all P<0.01)4.Correlation of serum Ma R1,CypD and other clinical parameters.In all subjects,serum Ma R1 levels were positively correlated with HDL-C and negatively correlated with WHt R,BMI,ALT,AST,GGT,UA,TG,FBG and Ty G index(all P < 0.05).In contrast,serum CypD levels were positively correlated with WHt R,BMI,DBP,GGT,TG,Ty G index and NFS score,and negatively correlated with HDL-C(all P < 0.05).5.logistic regression analysis of factors influencing T2 DM merging MAFLD.With whether or not MAFLD was combined as the dependent variable(no=0,yes=1)and Ma R1 and CypD as independent variables respectively,CypD was a risk factor for combined MAFLD in T2 DM and Ma R1 was a protective factor for combined MAFLD in T2 DM when uncorrected(model1);after correcting for adjustment variable 1(model 2),the correlations were unaffected;after correcting for the adjustment variable(model 2)on the basis of model 2 After correction for the adjustment variable(model 3),CypD remained an independent risk factor for the development of T2 DM combined with MAFLD,and Ma R1 remained an independent protective factor for the development of T2 DM combined with MAFLD(all P<0.05).6.ROC curve analysis.The area under the curve of serum Ma R1 and CypD for the diagnosis of T2 DM combined with MAFLD were 0.876(95% CI: 0.871-0.935)and 0.826(95% CI: 0.762-0.892),respectively;the best cut-off values for the diagnosis of T2 DM combined with MAFLD were 72.250 pg/ml and 5.590 ng/ml,respectively.The sensitivity was 71.2% and 71.6%,and the specificity was90.9% and 76.3%,respectively(all P<0.05).Conclusions:1.serum CypD levels were significantly increased and Ma R1 levels were significantly decreased in T2 DM patients with MAFLD compared to T2 DM.2.CypD is an independent risk factor for the development of T2 DM combined with MAFLD;Ma R1 is an independent protective factor for the development of T2 DM combined with MAFLD.3.CypD and Ma R1 both have good diagnostic efficacy for T2 DM combined with MAFLD.
