Objective:To study the level of postoperative quality of life in patients with low anterior rectal resection syndrome,to investigate the correlation between symptoms of low anterior resection syndrome and quality of life,and to analyze the factors affecting quality of life in patients with severe low anterior resection syndrome,so as to provide a theoretical basis for preventing low anterior resection syndrome and improving postoperative quality of life in patients with low anterior resection syndrome.Methods:Patients diagnosed with low rectal cancer and underwent low anterior resection of rectal cancer at the Affiliated Hospital of Chengde Medical College from April 2019 to April 2022 were followed up,and the LARS score scale and EORTC QLQ-C30 Quality of Life Questionnaire questionnaire were assessed to collect general information,clinicopathological data and treatment of patients,and 126 complete data were obtained.The correlation between LARS symptoms and quality of life was investigated,and the patients were divided into no/mild low anterior resection syndrome group and severe low anterior resection syndrome group by LARS scoring scale,and the differences in quality of life between the two groups were compared,and the factors affecting quality of life in patients with severe low anterior resection syndrome were further analyzed.Results:1.126 patients with low rectal cancer had LARS scores of 21.28±14.11,including 55 patients(43.65%)with no LARS,24 patients(19.05%)with mild LARS,and 47 patients(37.30%)with severe LARS,and the incidence of postoperative LARS in patients with low rectal cancer was 56.35%.2.The total score of LARS scale and each symptom score were negatively correlated with overall quality of life(P < 0.05).3.Comparing the quality of life of patients with no/mild LARS and patients with severe LARS,the differences between the two groups were statistically significant(P<0.05)in overall health/quality of life,somatic function,role function,emotional function,social function,fatigue,nausea and vomiting,pain,insomnia,diarrhea,and financial difficulties,and patients with severe LARS had lower quality of life.4.Univariate analysis showed statistically significant differences(p<0.05)in age(p=0.028),body mass index(p=0.042),tumor distance from the anal verge(p=0.02),and chemotherapy(p=0.023)in patients with severe low anterior resection syndrome.The results of multiple linear regression analysis showed that body mass index(p=0.006)and tumor distance from the anal verge(p=0.029)were independent influences on quality of life in patients with severe anterior resection syndrome(p<0.05).Conclusions:1.The postoperative quality of life of patients with low rectal cancer is closely related to the severity of LARS,and the quality of life of patients with severe low anterior resection syndrome is even lower.2.Body mass index and distance of the tumor from the anal verge are independent influences on quality of life in patients with severe low anterior resection syndrome.For patients with severe low anterior resection syndrome,maintaining a healthy body mass index should be a long-term goal to have a high quality of life. |
