| Objective:In elderly patients with cerebral infarction,its cognitive function and Helicobacter pylori infection(Helicobacter pylori,Hp),serum amyloid protease A(Serum amyloid proteinase A,SAA),8 hydroxydeoxyguanosine(8 hydroxydeoxyguanosine,8-OHdG),so the early to damage cognitive function risk factors and early intervention to provide some theoretical basis.Methods:Patients aged>≥65 and diagnosed with cerebral infarction from 2021 to June 2022 were randomly selected.The department should be general practice or geriatric diseases,and 180 cases included were the subjects.The Montreal Cognitive Assessment Scale for evaluating cognitive use(Montreal cognitive assessment,MoCA),Physicians need to have received professional training,And to refine the C13 breath test,According to the Hp results,The Hp-positive patients were set as the experimental group,Using the Hp-negative patients as a control group,General clinical data and high-density lipoprotein cholesterol(High-density lipoprotein cholesterol,HDL-C)and total cholesterol(Total cholesterol,TC)or triglycerides(Triglycerides,TG),ankle-brachial index(Ankle-humeral index,ABI),brachial and ankle pulse wave conduction velocity(Arm-ankle pulse wave conduction velocity,BaPWV),calculated by the formula(serum total cholesterol-high-density lipoprotein)/high-density lipoprotein arteriosclerosis index(Arteriosclerosis index,AI)class,Serserum of the enrolled patients,SAA as well as the levels of 8-OHdG,Software SPSS25.0 statistical analysis of the test results of the two groups,Studying the association of combined Hp infection with ABI,AI,baPWV index,SAA,8-OHdG level and its association with MoCA score,To explore the predictors associated with Hp infection and cognitive function in elderly patients with cerebral infarction.Results:1.Among the subjects of this study,there was a significant difference in the number of patients with cognitive impairment between the Hp positive group and the negative group(χ2=17.271,P<0.05),that is,the number of patients with cognitive impairment in the Hp positive group was significantly higher than that in the Hp negative group.It was also found that the cognitive function score of patients in the Hp positive group was lower than that in the Hp negative group.AI and baPWV in HP positive group were higher than those in negative group,and ABI was lower than those in negative group.The expression level of SAA and 8-OHdG in Hp positive group was higher than that in negative group,and the difference was statistically significant(P<0.05).2.Pearson correlation analysis showed that MoCA score was negatively correlated with age,blood pressure,TG,AI and baPWV(r1=-0.209,r2=-0.416,r3=-0.409,r4=-0.475,r5=-0.536,P<0.05),and MoCA score was positively correlated with years of education and ABI(r6=0.371,r7=0.384,P<0.05).3.Convert Hp infection into numerical data,take statistically significant factors as independent variables,and MoCA score as dependent variables.Multiple linear regression analysis shows that Hp positive,age,blood pressure and MoCA score are independently negatively correlated(B1=-1.109,B2=-0.061、B3=-0.152,P<0.05),and education years,ABI and MoCA score are independently positively correlated(B4=0.231,B5=0.286,P<0.05).4.The results of correlation analysis showed that the expression levels of SAA and 8-OHdG were positively correlated with AI(r1=0.674,r2=0.718,P<0.05).SAA and 8-OHdG levels were negatively correlated with MoCA scores(r1=-0.784,r2=-0.695,P<0.05).SAA and 8-OHdG were included in the multiple linear regression equation.After adjusting for Hp infection,age,years of education,ABI,baPWV,etc.,it was suggested that SAA and 8-OHdG levels were independently correlated with MoCA scores(B1=-0.149,B2=-0.126,P<0.05).Conclusions:1.Among the elderly patients with cerebral infarction,the degree of cognitive impairment in the Hp-positive group is significantly higher than that in the Hp-negative group.The higher the expression level of SAA and 8-OHdG,the more serious the cognitive impairment.2.Hp infection,old age,short education period,hypertension,low ABI,high AI and high baPWV are important risk factors for cognitive function of elderly patients with cerebral infarction.3.For elderly patients with cerebral infarction,it is recommended to improve the Hp infection test.For those with positive Hp,ABI,AI,baPWV should be checked as soon as possible,and cognitive function evaluation should be carried out as early as possible,as well as SAA,8-OHdG and other indicators should be measured,and risk factors should be found as early as possible,and intervention should be given to prevent the progress of cognitive dysfunction. |
