The Study On The Efficiency Of Different Image Enhancement Endoscopy On Detecting Colorectal Adenoma

Objective:The early detection and removal of colorectal polyps and adenomas is the most important thing to prevent colorectal cancer.Colonoscopy is a key way to detect colorectal polyps and adenomas.Image enhancement endoscopy(IEE)is a new type of optical endoscopy technology.In this study,three IEE modes((linked color imaging(LCI),Narrow-band imaging(NBI),and Optical enhancement(OE))were used to compare with white light.Polyp detection rate(PDR),Adenoma detection rate(ADR)and diagnostic accuracy of polyps in colon were compared.The differences of the above observation indexes among the three kinds of image enhancement endoscopy were compared.OE2 mode was chosen in this study because the color of mode 2 of OE was closer to white light and suitable for general survey.Methods:A total of 1400 patients who planned to undergo colonoscopy in the First Central Hospital of Baoding from October 2021 to October 2022 were selected.Subjects were randomly divided into four groups according to a randomized number table.There were 350 patients in each group(Finally,346 patients were included in the LCI group,347 patients in the NBI group,347 patients in the OE group,and 349 patients in the white light group).All patients successfully reached the ileocecal part and exited colonoscopy using different patterns according to the group.Polyps,adenomas,location and size of lesions were recorded.The white light group was used as the control group.PDR,ADR,polyp location and size,diagnostic accuracy and other indicators were compared with LCI,NBI and OE2.All the subjects wered also used to compare the differences among the three IEE groups.Results:1.The comparison of PDR in LCI group(48.6%),NBI group(46.1%)and OE2 group(45.2%)was higher than that in white light group(39.3%),but the difference between LCI group and white light group was statistically significant(P < 0.05).There was no significant difference among the three IEE groups(P > 0.05).2.Compared with the white light group,more micropolyps were detected in LCI group,NBI group and OE2 group(white light group 69.7%,LCI group 76.1%,NBI group 76.2%,OE2 group 75.4%).LCI group and NBI group were significantly different from white light group(P < 0.05).There was no significant difference among the three IEE groups(P > 0.05).3.The total ADR of LCI group was higher than that of white light group,the difference was statistically significant(40.1%vs.31.0%,P < 0.05).The difference was found mainly in the right colon(16.5%vs.10.6%,P <0.05)and transverse colon(12.7%vs.7.5%,P < 0.05).Compared with the three IEE groups,the total ADR of the LCI group was statistically different from that of the OE2 group(40.1%vs.31.4%,P < 0.017),and the transverse colon of the LCI group was statistically different from that of the OE2 group(12.7%vs.6.9%,P < 0.017).In the right colon,ADR in the LCI group was higher than that in the NBI group,with a statistically significant difference(16.5%vs.10.1%,P < 0.017).4.The diagnostic accuracy of LCI,NBI and OE2 were higher than that of white light,with a statistically significant difference(88.3%vs.79.5%;95.3%vs.79.5%;87.8%vs.79.5%;P < 0.05).Among the three IEE groups,the diagnostic accuracy of NBI group was higher than that of LCI group and OE2 group(95.3%vs.88.3%;95.3%vs.87.8%;P < 0.017).Conclusions:1.Compared with white light,PDR and ADR of LCI were higher.In particular,the ADR of LCI in the right colon and transverse colon were higher than those in white light.2.The ability of LCI and NBI to detect micropolyp is stronger than that of white light.3.The diagnostic accuracy of LCI,NBI and OE2 is higher than that of white light.4.Compared with LCI,NBI and OE2,LCI showed better performance in detecting adenoma.These advantages of LCI are more pronounced in the right colon and transverse colon.The diagnostic accuracy of colorectal polyps in NBI is higher.LCI,NBI and OE2 showed no significant difference in the size of polyps detected.
