The incidence rate of breast cancer is increasing every year in China,which is the first among female malignant tumors.Mammary Paget’s disease is a minor subtype of breast cancer(MPD).There are currently numerous disputes surrounding MPD,and more evidence-based medical evidence is required to improve MPD therapy.Purpose:To investigate the clinical and pathological aspects,as well as the prognosis of MPD.Methods:According to the inclusion criteria,the clinicopathological information and survival prognosis of MPD patients Collected in Qinhuangdao First Hospital between December 2012 and December 2022 For patients pathologically diagnosed with MPD from 2010 to 2019,clinical,pathological,and survival data were retrieved from The Surveillance,Epidemiology,and End Results(SEER)program database.The general data of patients in our hospital group and SEER group were analyzed by SPSS software.Kaplan-Meier performed survival analysis,and Log-Rank compared the survival difference between our hospital group and SEER group.Cox regression model was used for single-factor and multi-factor consequence analysis.In this paper,statistical significance was defined as P<0.05.Results:The age of onset for the 651 MPD patients included in this study varied from 24 to 85 years,with an average age of(60.61±14.43)years.Of these,35 patients were enrolled in the hospital group and 616 were involved in the SEER group.Patients in our hospital group primarily had changes to their areolas and nipples that resembled eczema at first,ulceration and crusting(17/35,48.57%),all patients underwent surgical treatment,Simple mastectomy was performed in 11 cases(11/35,31.4%)and modified radical mastectomy was performed in 24 cases(24/35,68.57%).Among the pathological types,3patients(8.57%)had MPD alone,15 patients(42.86%)had MPD combined with intraductal carcinoma,and 17 patients(48.57%)had MPD combined with invasive carcinoma.The positive rates of ER,PR and HER2 were 28.57%,14.29% and 80%,respectively,HER2 positive was the most common immunohistochemical subtype.There were 144 instances in the SEER group where the age of onset was primarily confined to the 50-59 age range.277patients(277/616,44.97%)underwent simple mastectomy.The positive rate of ER,PR and HER2 in SEER group was 55.19%,38.47% and 64.77%(399/616).HER2 positive breast cancer was the most common immunohistochemical subtype.the group as a whole had an average survival time of(111.88 + 5.41)months.The overall survival rate at 1 year and 3 years is 100%,and the overall survival rate at 5 years is 94.29%.disease-free survival rates were 100%,94.29% and 91.43%,respectively.The mean survival time of SEER group was(96.00±1.57)months,The 1 year overall survival rate was 96.59%,the 3 year overall survival rate was 87.5%,and the5 year overall survival rate was 79.70%.Regarding survival analysis,Univariate analysis showed that age,surgical procedure,T stage,N stage and M stage were significantly correlated with survival state of MPD patients(P < 0.05),Multivariate survival analysis suggested that age,T,N,and M stages were independent prognostic factors for death in MPD patients.Conclusion:The onset age of MPD was mainly in middle-aged and elderly women,the first symptom is mostly nipple and areola eczema like change,rupture,scab,Simple MPD was the least common pathological type,most of the immunohistochemical types were HER2 positive,the overall prognosis is good.
