Objective:Clinical efficacy of modified Fu Rong Tong Mai Tang on patients with diabetic peripheral neuropathy with Qi and Yin deficiency and stasis.Comparison of changes in insulin-like growth factor-1 in two phases before and after treatment.Observation of the clinical efficacy and preliminary investigation of the possible mechanism of action of Fu Rong Tong Mai Tang in the treatment of diabetic peripheral neuropathy.Provide theoretical basis for clinical application.Methods:Sixty-six patients with DPN who met the inclusion criteria were divided into a control group and a test group according to the random number table method.33 cases of each,Both groups were given basic treatment, while the control group received oral epalrestat tablets,50mg/d,3times/d.The experimental group combined with the control group on the basis of the modified Fu Rong Tong Mai Tang,1 sachet twice a day,200 ml of boiling water each time.Treatment for 4 weeks is 1 course of treatment for a total of3 courses of treatment. The nerve conduction velocity,Changes in serum IGF-1 levels,TCM evidence scor,e Toronto clinical neuropathy score and other clinical indicators,To evaluate the effect of modified Fu Rong Tong Mai Tang in the treatment of DPN,The data collected in this study were statistically analyzed at the end of the treatment using SPSS 26.0 software.Results:1.The speed of bilateral common peroneal nerve conduction velocity(MNCV,SNCV)improved significantly after treatment,(P<0.05),and the effect of the test group was significantly better than that of the control group(P<0.05),and the difference was statistically significant.Bilateral tibial nerve conduction velocities were slightly increased in both groups after treatment compared with those before treatment, but the difference was not statistically significant(P>0.05).2.After treatment, the TCM evidence points and TCSS scores decreased in both groups(P<0.05),and the test group had better results compared with the control group, with statistically significant differences(P<0.05).3.After 12 weeks of treatment, the efficacy of the two groups was higher than that of the control group, and the difference was statistically significant(P<0.05).4.Serum IGF-1 levels increased in both groups after treatment(P<0.05),and the test group showed significantly better results than the control group. The differences were statistically significant(P<0.05).5.The general baseline comparison between the two groups(P>0.05)was comparable.Fasting blood glucose,2-hour postprandial blood glucose, and glycated hemoglobin test were at similar levels before and after treatment in both groups.Total cholesterol, triglycerides, and LDL cholesterol decreased in both groups.HDL cholesterol was elevated, and the difference was not statistically significant(P>0.05).Conclusions:1.Modified Fu Rong Tong Mai Tang can improve the nerve conduction velocity, reduce the TCM evidence score and TCSS score, and improve the clinical efficacy in patients with DPN with Qi and Yin deficiency and stasis.2.The Modified Fu Rong Tong Mai Tang can increase the IGF-1 level and improve the clinical symptoms in patients with DPN with Qi-Yin deficiency and stasis,so it is presumed that the modified Fu Rong Tong Mai Tang can exert therapeutic effects on DPN with Qi-Yin deficiency and stasis by affecting the IGF-1 expression level.
