The Construction Of PCCM Nurse Standardized Training Program Based On Mini-CEX

Objective: This study referred to Miller’s ability pyramid model,Waston’s humanistic Care theory and nursing process theory,combined with the teaching objectives of standardized training for nurses in China and the professional requirements of Pulmonary and Critical Care Medicine(PCCM)for nurses.To construct a scientific and comprehensive standardized training program for PCCM nurses on the basis of MiniClinical Evaluation Exercise(Mini-CEX),so as to provide a reference for training clinical nurses in Pulmonary and Critical Care Medicine.Methods: Literature review and semi-structured interviews were used to initially construct the first draft of the standardized training outline for PCCM nurses based on Mini-CEX.Delphi expert consultation was used to investigate 22 nursing experts.EXCEL database was created,and SPSS25.0 software was used to statistically analyze the consultation results.Analytic Hierarchy Process was used to determine the weight of the training outline content,and the reliability and validity were tested.Finally,the standardized training outline for PCCM nurses based on Mini-CEX was formed.Results: 1.The positive coefficients of the two rounds of consultation were 100%and 95%,the authority coefficients were 0.915 and 0.925,the coordination coefficients of expert opinions were 0.169 and 0.167,P < 0.05,and the variation coefficients of the two rounds of consultation were 0.072-0.277 and 0.044-0.169.It shows that the enthusiasm,authority and representativeness of the experts are high,and the opinions of the experts are basically consistent.2.After two rounds of expert consultation,the standardized training outline of PCCM nurses based on Mini-CEX was finally determined with three training modules of specialized knowledge,specialized skills and attitude,including 7 training items,52 training contents,and clear training objectives,training methods and hours.The weights of training items were 0.144,0.141,0.149,0.146,0.140,0.136 and 0.143,among which nursing diagnosis and measures were the most important.3.The Cronbach’s α coefficients of training items and training contents in the second round of consultation were 0.861 and 0.977,and the split-half coefficients were 0.834 and0.857,respectively.The test-retest reliability analysis results showed significant correlation,indicating that the reliability of the syllabus was good.The content validity analysis results showed that the correlation was significant,and the KMO sampling appropriateness was 0.707.The exploratory factor analysis results showed that the outline had a clear structure,indicating that the outline had good reliability and validity.Conclusions: The standardized training outline for PCCM nurses based on MiniCEX constructed in this study has good reliability and validity.The content of the outline is prominent,focusing on the cultivation of specialized knowledge,specialized skills and attitudes.The training content is comprehensive and reliable,which can provide a training outline that conforms to the characteristics of respiratory diseases and is suitable for clinical nursing teaching for the standardized training of nurses in the department of respiratory and critical care medicine.
