Objective: To analyze the risk factors for postoperative complications of the da Vinci robotic esophageal cancer Mc Keown procedure.Methods: One hundred and ninety-three patients with esophageal cancer who underwent robotic Mc Keown surgery at the First Hospital of Lanzhou University from April 2018 to August 2022 were retrospectively included.Among them,159 were male and 34 were female,with a mean age of(61.96±7.83)years,and were divided into observation group(n=117)and control group(n=76)according to whether postoperative complications occurred,and one-way test and multi-factor logistic regression analyses were performed for risk factors related to postoperative complications affecting postoperative complications.Results: All 193 patients successfully completed radical oesophageal cancer surgery without intraoperative death,and the choking sensation of eating basically disappeared after surgery.There were 12 cases of oesophageal anastomotic fistula,40 cases of anastomotic stenosis,67 cases of pulmonary infection,48 cases of pleural effusion,4 cases of gastro-oesophageal reflux disease,8 cases of pulmonary atelectasis,10 cases of arrhythmia and 10 cases of heart failure.Multi-factor logistic regression analysis showed that: BMI [OR=0.765,95% CI(0.644,0.908),chronic respiratory history [OR=2.495,95% CI(1.269,4.905),P=0.008],postoperative albumin[OR=0.902,95% CI(0.820,0.993),P=0.036],and time to surgery [OR=1.006,95% CI(1.002,1.016),P=0.037] were robotic predictive risk factors for postoperative complications in oesophageal cancer.Conclusion: The presence of postoperative complications in robotic oesophageal cancer Mc Keown surgery was closely associated with perioperative indicators,with preoperative BMI <22.71 kg/m^2,a history of chronic respiratory disease,postoperative albumin <33.15 g/L,and operative time >255.52 min being independent risk factors for postoperative complications in oesophageal cancer Mc Keown.Clinicians can implement targeted prevention and treatment based on the patient’s BMI,history of chronic respiratory disease,postoperative albumin level,and time to surgery.
