| Objective:Taking the hospitalization expenses of the six common chronic diseases of the elderly as the research object,this paper analyzes the composition of the hospitalization expenses of the common chronic diseases of the elderly,the economic burden of hospitalization of various chronic diseases,and explores the sensitivity of various influencing factors to the hospitalization expenses based on BP neural network model,providing theoretical support for the management of chronic diseases of the elderly,and providing reference for the formulation of policies to reduce the burden of chronic diseases of the elderly.Methods:The medical record information system of a third class A general hospital in Anhui Province was retrieved retrospectively from January 2018 to December 2020,and with the age of hospitalization≥60 years old and hospitalized for six common chronic diseases.The study used descriptive methods such as frequency,constituent ratio,median and interquartile interval(P25~P75)to analyze the general situation and basic situation of hospitalization in the first page of inpatient medical records of common chronic diseases in the elderly.SPSS chi square test was used for single factor analysis,and statistically significant(P<0.05)was included in the BP neural network model for in-depth analysis.Results:1.The composition of hospitalization expenses for common chronic diseases of the elderly.The items of hospitalization expenses for chronic diseases of the elderly in this study include nursing expenses,medicine expenses,examination expenses,consumables expenses,and treatment expenses.Among them,the drug expenses of3081.2 yuan(1460.2~5965.0)are the most expensive,followed by the examination expenses of 2747.5 yuan(1567.0~4306.5).2.Analysis of hospitalization expenses for various chronic diseases of the elderly.Among the hospitalization expenses,14227.2(6921.3~78826.7)were chronic diseases of the respiratory system,12735.4(7031.6~1987.1)were malignant tumors,and11591.2(5980.2~47886.5)were cerebrovascular diseases;Among all chronic diseases,the drug expense was the most expensive item,followed by the inspection expense,among which the drug expense of respiratory chronic diseases was 3697.2(1685.9~7689.4);Among the examination costs,diabetes 2947.3(1697.2~4442.9)costs the most,treatment costs 895.0(375.0~2474.5)and nursing costs 442.0(255.0~902.7)costs the most for respiratory diseases,and consumables costs 405.0(139.0~1861.5)costs the most for hypertension.3.Single factor analysis of the influencing factors of chronic disease hospitalization expenses of the elderly.The single factor analysis of the elderly’s general situation on the hospitalization expenses of chronic diseases found that the expenses were statistically significant in the annual distribution,gender,age,occupational status,and medical insurance type(P<0.05);In the univariate analysis of hospitalization of the elderly on the hospitalization expenses of chronic diseases,the number of days in hospital,whether to operate,the way of admission,drug allergy,admission and discharge were statistically significant(P<0.05);The influence of 6 kinds of chronic diseases on hospitalization expenses of the elderly was statistically significant(P<0.05).4.BP network model analysis of the elderly chronic disease hospitalization expenses.Based on the BP network model analysis,it is found that the number of days in hospital(100%),age(73.6%),and surgery(37.0%)are the main factors affecting the hospitalization expenses.Among the chronic diseases,respiratory diseases(30.3%),cerebrovascular diseases(28.2%),and heart diseases(25.7%)have the largest impact on the hospitalization expenses.Conclusion:1.The hospitalization cost of common chronic diseases of the elderly is high and the economic burden is heavy2.The number of days and age of hospitalization for six common chronic diseases in the elderly are the first and second major influencing factors for high hospitalization costs,and they increase with the increase of hospitalization days and age in the elderly.3.The drug expense is the highest among the hospitalization expenses for various chronic diseases,and the hospitalization expense for chronic respiratory diseases is the highest. |
