Effect Of Posterior Nasal Neurectomy With Pharyngeal Neurectomy On Chronic Sinusitis With Nasal Polyps Complicated With Allergic Rhinitis

Objective To investigate the effect of posterior nasal neurectomy with pharyngeal neurectomy(PNN+PN)on chronic sinusitis with nasal polyps complicated with allergic rhinitis.Methods Eighty-three patients with perennial allergic rhinitis combined with chronic group-wide sinusitis with nasal polyps who attended our hospital from July 2020 to July 2021 were selected.All patients underwent conventional functional endoscopic sinusitis surgery(FESS)+ nasal polypectomy.According to whether the patients underwent PNN+PN,the patients were divided into: 44 patients in the control group who did not undergo PNN+PN on the basis of conventional surgery,and 39 patients in the experimental group who underwent PNN+PN on the basis of conventional surgery.All patients underwent the VAS,RQLQ,SNOT-22 and MLK before treatment,and at 6 months and1 year after surgery.Meanwhile,other relevant data,the preoperative and postoperative follow-up data were collected and analysed to assess the differences between the two groups.Results The total post-operative follow-up period was 1 year.The recurrence rate of nasal polyps at 1 year postoperatively and the nasal congestion VAS score at 6 months postoperatively were not statistically significant in the two groups(p > 0.05).However,the patients in the experimental group had statistically significantly lower effusion,sneezing and pharyngeal symptom VAS scores,MLK endoscopy scores,RQLQ and SNOT-22 scores at 6 months and 1 year postoperatively,and nasal congestion VAS scores at 1 year postoperatively compared to the control group(p <0.05).In the 1-year follow-up,it was found that 1 case of active bleeding,1 case of headache and 1 case of nasal dryness occurred in the experimental group,2 cases of postoperative headache and 3 cases of minor bleeding occurred in the control group,and no complications such as dry eyes and numbness of the upper lip nerve occurred in all patients.Conclusion For the patients with perennial AR complicated with CRSw NP,resection of the posterior nasal nerve and pharyngeal branch during nasal endoscopic sinus surgery can significantly improve the short-term curative effect.
