Objective:The distribution of TCM syndrome elements in polycystic ovary syndrome(Polycystic Ovary Syndrome,PCOS),the mental state and risk factors of PCOS patients were analyzed,and the correlation between TCM syndrome elements and abnormal mental state factors in PCOS patients was explored,providing more ideas for PCOS conditioning in TCM treatment,psychological intervention,prevention of high-risk groups and other aspects.Methods:A cross-sectional epidemiological survey was conducted.A total of 260 PCOS patients who met the inclusion criteria in the gynecological clinic of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from November 2021 to January 2023 were collected.The general information questionnaire,PCOS TCM syndrome elements questionnaire and SCL-90 symptom self-rating scale were collected.All statistical data were entered into SPSS 26.0 software for spearman correlation analysis,Logistic regression analysis and other statistical analysis methods.The general information distribution of PCOS patients,the distribution law of PCOS syndrome elements,the abnormal psychological state factors of PCOS patients,the correlation analysis between the abnormal psychological state factors of PCOS patients and TCM syndrome elements,and the related risk factors of PCOS were obtained.Results:1.A total of 260 cases were available in this study,of which the age was between 16 and 35 years old,the most involved occupations were students,and the highest proportion of education level was undergraduate.The age of menarche was mostly concentrated in 11 to 14 years old,and 45.4% of the patients could have regular menstruation within one year after menarche.However,20.4% of the PCOS patients did not have regular menstruation for two years after menarche,and 64.6% of the PCOS patients had a body mass index(BMI)higher than the normal range(18.5-24).38.5% of the patients stayed up late and worked at night.Most PCOS patients preferred spicy food(56.2%).66.5% of the patients never exercised.2.In PCOS patients,the proportion of disease location syndrome factors from high to low was 65.0% in liver,63.5% in uterus,55.0% in spleen,36.9%in kidney,33.8% in stomach,14.2% in heart,10.4% in lung and 7.7% in gallbladder.The top three syndrome factors in combination were liver +uterus(55.0%),liver + spleen(30.4%)and liver + kidney(29.6%).The proportion of deficiency syndromes from high to low was qi deficiency 66.2%,Yang deficiency 41.2%,Yin deficiency 30.8%,and blood deficiency 17.3%.The proportion of empirical syndrome factors from high to low was: blood stasis 43.1%,qi stagnation 40.4%,dampness 37.3%,phlegm 37.3%,heat36.5%,Yang hyperthyroidism 13.1%.The top three combination of deficiency+ empirical excess disease were: qi deficiency + heat 36.2%,qi deficiency + qi stagnation 29.6%,and qi deficiency + dampness 26.2%.The most common combination of disease location and disease was liver + uterus+ qi deficiency + qi stagnation(22.7%),spleen + stomach + dampness + heat(20.4%),liver + uterus + qi deficiency + dampness(17.3%).3.Among 260 PCOS patients,there were 200 patients with abnormal psychological state,and the distribution of abnormal factors was as follows:23.0% for somatization,74% for compulsion,37.0% for depression,34.5%for anxiety,38.5% for interpersonal sensitivity,25% for phobia,38.0% for hostility,29% for paranoia,21.0% for psychosis,and 51.2% for others.4.The correlation results between PCOS syndrome elements and abnormal mental state factors were as follows: compulsion was positively correlated with uterus,and blood stasis,but negatively correlated with stomach and heat.The sensitivity of interpersonal relationship was positively correlated with spleen,qi deficiency,phlegm and dampness,and negatively correlated with stomach.Depression was positively correlated with spleen,phlegm and dampness,but negatively correlated with stomach.Anxiety and hostility are positively correlated with liver,qi deficiency,Yin deficiency,qi stagnation and heat.5.In the family history of PCOS patients,regression analysis showed that abnormal glucose and lipid metabolism in first-degree relatives was a risk factor for BMI≥28 in PCOS patients.Conclusion:1.Liver,uterus,spleen and kidney were the most common site syndromes of PCOS,and uterus and liver were the most common compound site syndromes.The main syndromes of deficiency diseases were qi deficiency and Yang deficiency.The main empirical syndrome factors solid disease were blood stasis,qi stagnation,dampness,phlegm and heat.The combination syndrome of deficiency and empirical mainly included qi deficiency + heat,qi deficiency + qi stagnation,qi deficiency + dampness.The complex syndrome of disease location and disease mainly included liver+ uterus + qi deficiency + qi stagnation,spleen + stomach + heat + dampness,liver + uterus + qi deficiency + dampness.2.The factors of abnormal mental state in PCOS patients are mostly obsessive-compulsive,interpersonal sensitivity,hostility,depression and anxiety.3.The syndrome elements of PCOS were closely related to the factors of abnormal mental state,mainly as follows: compulsion was positively correlated with uterus and blood stasis,but negatively correlated with stomach and heat;The sensitivity of interpersonal relationship was positively correlated with spleen,qi deficiency,phlegm and dampness,and negatively correlated with stomach.Depression was positively correlated with spleen,phlegm and dampness,but negatively correlated with stomach.Anxiety and hostility are positively correlated with liver,qi deficiency,Yin deficiency,qi stagnation and heat.4.Using BMI≥28 as a reference,PCOS patients with abnormal glucose and lipid metabolism in first-degree relatives were more likely to be obese.
