Effect Of Electroacupuncture Combined With Exercise Training On The Expression Of NLRP3 Inflammatory Vesicles And Its Related Factors In Rats With Myocardial Ischemia/Reperfusion Injury

Objective:The effects of electroacupuncture combined with exercise training intervention method on cardiac function,myocardial infarct area,myocardial histopathological morphology,NLRP3 inflammatory vesicles and related inflammatory factors expression levels in rats with myocardial ischemia/reperfusion injury were observed.Methods:In this experiment,90 clean grade SD male rats were selected and randomly divided into control group(Con group),model group(IR group),electroacupuncture+model group(EA+IR group),exercise+model group(EP+IR group)and electroacupuncture combined with exercise+model group(EA+EP+IR group),18 rats in each group.Myocardial ischemia-reperfusion injury model was established using Langendorff isolated perfusion system.180 min of equilibrium perfusion was applied to the control group after heart isolation,and 20 min of equilibrium perfusion,then 40 min of ischemia,and finally 120 min of reperfusion were applied to the model group,electroacupuncture group,exercise group and combined group after heart isolation.the electroacupuncture group used Yu Yuan matching acupuncture point method to take bilateral Heart Yu and Shen Men acupuncture points.The electroacupuncture intervention was performed 20min/time daily for 6d per week for 3 weeks.In the exercise group,the gradient training of the electric running table was used for 3 weeks,and the training was conducted once a day for 6 d.The intervention method and duration of the combined group were the same as those of the electroacupuncture group and the exercise group.During the experiment,cardiac function was monitored;TTC staining was used to measure the area of myocardial infarction;HE staining was used to observe the pathological morphology of myocardial tissues of rats;enzyme-linked immunosorbent assay(ELISA)was used to detect the content of IL-1 β and IL-18 in myocardial tissues;protein blotting(Western Blot)was used to detect the protein expression levels of NLRP3,caspase-1,IL-1βand IL-18 protein expression levels in myocardial tissue.Results:1.Comparison of cardiac function indexes of rats in each group.Compared with the Con group,all the cardiac function indexes were significantly lower in the IR group(P< 0.05);Compared with the IR group,the cardiac function indexes were significantly higher in each intervention group,and the function was improved(P< 0.05);The increase was most obvious in the EP+EA+IR group and in the intervention group(P< 0.05).2.Myocardial infarct area expression by TTC staining.There was no myocardial ischemia and infarction in the Con group.Compared with the Con group,the myocardial infarct area was substantially increased in the IR group(P< 0.05);Compared with the IR group,the myocardial infarct area was reduced in all intervention groups(P< 0.05);Among the intervention groups,the myocardial infarct area was reduced more significantly in the EP+EA+IR group(P< 0.05).3.Histomorphological changes of myocardial tissue of rats in each group:Myocardial histomorphological changes were observed under microscope(400×)in all groups of rats.No significant pathomorphological changes in myocardial tissue of rats in the Con group;myocardial tissue in the IR group showed fractured and swollen fibers,disorganized arrangement,disrupted cell structure and uneven staining,visible hemorrhage and necrotic areas,and a large number of inflammatory cell infiltrates;compared with the IR group,myocardial tissue in the EA+IR and EP+IR groups showed partially fractured fibers and Compared with the IR group,the EA+IR and EP+IR groups had partially broken myocardial tissue fibers,slightly neat arrangement,incomplete cell structure,and more interstitial inflammatory cell infiltration;the EP+EA+IR group had lighter myocardial fiber edema,basically neat arrangement,more complete myocardial cell structure,but still had a little interstitial inflammatory cell infiltration,and the degree of pathological damage was significantly reduced compared with the IR,EA+IR and EP+IR groups.4.Contents of IL-1β and IL-18 in myocardial tissues of rats in each group.The contents of inflammatory factors IL-1β and IL-18 in myocardial tissues of rats in the Con group were lower compared with all other groups,and the differences were statistically significant(P< 0.05).The contents of IL-1β and IL-18 in myocardial tissues were increased in the IR group compared with the Con group,and the differences were statistically significant(P< 0.05).Compared with the IR group,the contents of IL-1 β and IL-18 in myocardial tissue decreased in each intervention group(P< 0.05);Among the intervention groups,the contents of IL-1 β and IL-18 in myocardial tissue decreased more significantly in the EA+EP+IR group(P< 0.05).5.The protein expression levels of NLRP3,caspase-1,IL-1β,and IL-18 in myocardial tissues of rats in each group.The expression levels of NLRP3,caspase-1,IL-1β,and IL-18 proteins in myocardial tissues of rats in the Con group were lower than those in the remaining three groups.The expression levels of NLRP3,caspase-1,IL-1β,and IL-18 proteins in myocardial tissues were increased in the IR group compared with the Con group(P< 0.05).The expression levels of NLRP3,caspase-1,IL-1β,and IL-18 protein in myocardial tissues were decreased in each intervention group compared with the IR group(P< 0.05).Among the intervention groups,the decrease of NLRP3,caspase-1,IL-1β,and IL-18 protein expression levels in myocardial tissues was more significant in the EA+EP+IR group(P< 0.05).Conclusion:1.Electroacupuncture combined with exercise training improves cardiac function in rats with myocardial ischemia/reperfusion injury.2.Electroacupuncture combined with exercise training reduces the degree of myocardial histomorphological damage and exerts a cardioprotective effect.3.Electroacupuncture combined with exercise training may exert myocardial protective effects by acting on NLRP3 inflammatory vesicles and regulating the expression of NLRP3,caspase-1,IL-1β and IL-18 in myocardial tissue of rats with myocardial ischemia/reperfusion injury.
