| Objective: The purpose of this study was to investigate the effect of acupuncture combined with myoelectric biofeedback therapy on lower limb functional rehabilitation of patients with hemiplegia after stroke.The purpose of this study is to explore the effect of electromyographic biofeedback combined with acupuncture therapy on the rehabilitation of lower limb function in patients with hemiplegia after stroke.Methods: 105 patients with stroke hospitalized in the Department of Rehabilitation Medicine of the First Affiliated Hospital of Bengbu Medical College from January 2021 to October 2022 were selected as the study subjects.They were divided into acupuncture group,electromyographic biofeedback group and combined group according to the method of random number table,with 35 patients in each group.All patients in the three groups received routine rehabilitation treatment,the acupuncture group received acupuncture treatment,the electromyographic biofeedback group received electromyographic biofeedback treatment,and the combined group received acupuncture and electromyographic biofeedback treatment.Once a day,5 days a week,and the treatment time is 4 weeks.Active range of motion(AROM),Fugl-Meyer assessment of lower extremity motion(FMA-LE),Berg Balance Scale(BBS),Holden walking function grading,Modified Barthel Index(MBI)The modified Ashworth Scale(MAS)and the surface electromyography(integrated electromyogram,i EMG,root mean square value,RMS)of the tibialis anterior muscle,peroneus longus and peroneus longus muscle and gastrocnemius muscle of the three groups of patients were tested.Results: There was no statistically significant difference(P>0.05)in the general data of gender,age,course of disease,stroke type,and hemiplegic side among the three groups of study subjects.No adverse events occurred during the intervention period among the three groups of patients.(1)Before treatment,there was no significant difference in AROM scores among the three groups(P>0.05),which was comparable.Intra-group comparison: After 4 weeks of treatment,AROM scores of the three groups were improved compared with those before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of AROM score in the combined group was better than that in the electromyography biofeedback group and the acupuncture group,with statistical significance(P<0.05),but there was no statistical significance in the acupuncture group compared with the electromyography biofeedback group(P>0.05).(2)Before treatment,there was no statistical significance in Fugl-Meyer motor function scores of lower limbs among the three groups(P>0.05),which was comparable.Intra-group comparison:After 4 weeks of treatment,the lower limb Fugl-Meyer motor function score of the three groups was improved compared with that before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of lower limb Fugl-Meyer motor function score in combination group was better than that in electromyography biofeedback group and acupuncture group,with statistical significance(P<0.05),but there was no statistical significance in comparison between acupuncture group and electromyography biofeedback group(P>0.05).(3)Before treatment,there was no statistical significance in the BBS scores of the three groups(P>0.05),which was comparable.Intra-group comparison: After 4 weeks of treatment,the BBS scores of the three groups were improved compared with those before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of the BBS score combined group was better than that of the electromyography biofeedback group and the acupuncture group,with statistical significance(P<0.05),but there was no statistical significance between the acupuncture group and the electromyography biofeedback group(P>0.05).(4)Before treatment,there was no significant difference in Holden walking capacity among the three groups(P>0.05),indicating that the three groups were comparable.Intra-group comparison:After 4 weeks of treatment,Holden walking capacity of the three groups was improved compared with that before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of Holden walking ability in the combined group was better than that in the electromyography biofeedback group and the acupuncture group,with statistical significance(P<0.05),but there was no statistical significance in the acupuncture group compared with the electromyography biofeedback group(P>0.05).(5)Before treatment,there was no significant difference in MBI among the three groups(P>0.05),indicating comparability.Intra-group comparison: After 4 weeks of treatment,MBI in all three groups was improved compared with before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of patients in the MBI combination group was better than that in the electromyography biofeedback group and the acupuncture group,with statistical significance(P<0.05),but there was no statistical significance in the acupuncture group compared with the electromyography biofeedback group(P>0.05).(6)Before treatment,there was no statistical significance in MAS between the three groups(P>0.05),indicating comparability.Intra-group comparison: After 4 weeks of treatment,MAS in all three groups was improved compared with before treatment,the difference was statistically significant(P<0.05).Comparison between groups: After 4 weeks of treatment,the improvement of MAS combined group was better than that of electromyography biofeedback group and acupuncture group,the difference was statistically significant(P<0.05),but there was no statistically significant difference between acupuncture group and electromyography biofeedback group(P>0.05).(7)Before treatment,there was no statistical significance in i EMG and RMS of tibialis anterior muscle,peroneus long or short muscle and gastrocnemius muscle among the three groups(P>0.05),showing comparability.Intra-group comparison: After 4 weeks of treatment,i EMG and RMS scores of tibialis anterior muscle,peroneus long or short muscle and gastrocnemius muscle in 3 groups were improved compared with those before treatment,with statistical significance(P<0.05).Comparison between groups:After 4 weeks of treatment,i EMG and RMS of tibialis anterior muscle,peroneus long or short muscle and gastrocnemius muscle in combination group were better than those in electromyographic biofeedback group and acupuncture group,with statistical significance(P<0.05),but there was no statistical significance between acupuncture group and electromyographic biofeedback group(P>0.05).Conclusion: Electromyography biofeedback combined with acupuncture can improve the lower limb motor function of stroke patients in recovery period,improve the active range of motion of ankle dorsiflexion joint,enhance the balance function,reduce lower limb muscle spasm,and improve the walking ability of patients. |
