Background and objectiveCurrent research suggests that non-small cell lung cancer(NSCLC)is closely related to chronic obstructive pulmonary disease(COPD),a risk factor for lung cancer.Studies have shown a high rate of missed diagnosis of COPD in lung cancer patients,and the clinical characteristics associated with COPD in NSCLC patients remain unclear.The purpose of this study was to investigate the prevalence and missed diagnosis rate of COPD in patients undergoing NSCLC surgery,and to clarify the clinical characteristics of patients with NSCLC complicated with COPD.MethodsThis study retrospectively analyzed the data of patients with NSCLC who underwent surgical treatment for lung cancer in the Department of Thoracic Surgery of Our hospital from October 2019 to December 2021.Statistical data were collected on COPD in patients after NSCLC surgery.According to whether patients with COPD were complicated after NSCLC surgery,the patients were divided into two groups:NSCLC complicated with COPD and simple NSCLC.Gender,age,BMI value,smoking history,pathology,lung function,blood routine,coagulation function and other indicators were compared between the two groups.Results1.Among 1401 lung cancer patients,there were 183 cases of COPD,the prevalence rate was 13.06%,only 9 cases(4.92%)were diagnosed with COPD during hospitalization,and the missed diagnosis rate of COPD in lung cancer patients was95.08%.Diffusion dysfunction and small airway dysfunction in 1401 lung cancer patients were 423(30.19%)and 701(50.04%),respectively.2.The proportion of clinical symptoms of lung cancer combined with COPD was significantly higher than that of lung cancer alone.Among them,cough,sputum with blood or hemoptysis,fatigue in lung cancer combined with COPD group is more common。3.The age,smoking history ratio and emphysema ratio of lung cancer combined with COPD group were higher than those of simple lung cancer group.The BMI of lung cancer combined with COPD was lower than that of lung cancer alone.There were more males than females in lung cancer patients complicated with COPD.4.The proportion of squamous cell carcinoma in COPD group was higher than that in lung cancer group.5.APTT,FIB,D-D and other coagulation indexes of lung cancer combined with COPD group were higher than those of lung cancer group alone.The WBC,NEUT,NEUT%,EO and EO% of lung cancer patients in COPD group were higher than those in lung cancer group alone.The Pa O2 and Sa O2 of lung cancer combined with COPD were lower than those of lung cancer alone.Conclusions1.The diagnosis rate of non-small cell lung cancer combined with COPD is low and the rate of missed diagnosis is high.When the lung function of most patients is significantly decreased,no further examination and treatment are performed.2.Patients with non-small cell lung cancer combined with COPD were older,had a higher percentage of smokers,had a higher percentage of squamous cell carcinoma,and had more severe clinical manifestations than patients with simple non-small cell lung cancer.
