| Objective: To use photoelectric pulse sensor to detect pulse wave,analyze the effective features of pulse waveform extraction,evaluate the degree of non-invasive arteriosclerosis in the human body through pulse wave characteristic parameters,and judge the cardiovascular health status,so as to provide a reference for clinicians in medical departments to detect arteriosclerosis in early stage,timely intervention and treatment,and promote the improvement of community medical services.Methods: The photoelectric pulse wave sensor is used to collect the volumetric pulse wave signal of the human finger end,the signal is converted into analog-to-digital and the digital pulse signal is sent to the PC through the serial port,and the pulse wave signal is analyzed by the pulse signal analysis system developed based on Lab VIEW as the platform.Firstly,the signal is preprocessed(denoising and removing baseline drift,etc.),and then the feature points are identified and the time parameters are extracted from various characteristic parameters such as the time between the main wave and the falling isthmus,the time between the main wave peak and the reflected wave crest T2(PPT),the time between the falling middle isthmus and the reflected wave crest,the main peak time T4(CT),the descending branch slope S1,S2,S3 and the ascending branch slope S4,morphological characteristic reflection index(RI),K and b/a and other characteristic parameters and statistical correlation analysis.Results:(1)The comparison of PPG characteristic parameters between healthy youth group and arteriosclerosis group(Table 3)showed that there were significant differences between K,CT,PPT,RI,S1,S2,S3,S4,T1,T3 and b/a between healthy youth group and arteriosclerosis group(P<0.05),and there were significant differences between K,CT,PPT and S4 in mild and severe arteriosclerosis group(P<0.05).There was a significant difference between IMT values(P<0.001)between mild and severe arteriosclerosis groups,and IMT was significantly correlated with PPG characteristic parameters K,CT,PPT,S3,S4,b/a,and negatively correlated with PPT.IMT was positively correlated with PPG characteristic parameters K(r = 0.780,P<0.01),CT(r = 0.653,P<0.01),and negatively correlated with PPT(r =-0.651,P<0.01),S1(r =-0.459,P<0.05),S4(r =-0.464,P<0.05),b/a(r =-0.417,P<0.05).In the logistic regression analysis for predicting arteriosclerosis,the area under the ROC curve(AUC)of the PPG characteristic parameters K,CT,PPT,RI and S4 for the prediction of arteriosclerosis was 0.917,0.902,0.995,0.944,0.718(P<0.01),;PP and the area under the ROC curve(AUC)of the characteristic parameters K,CT,PPT and S4 to assess the degree of arteriosclerosis lesions was 0.726,0.919 and 0.821,respectively,0.686(P<0.01),IMT has an area under the ROC curve(AUC)of 0.895.(2)There were differences in the morphology of pulse wave waveforms of different ages,and the differences between the characteristics of the young group and the middle-aged group and the elderly group were statistically significant.There was a significant correlation between age and characteristic parameters,and age was positively correlated with PPG characteristic parameters K(r =0.822,P<0.01),CT(r = 0.439,P<0.01),RI(r = 0.525,P<0.01),and positively correlated with PPT(r =-0.636,P<0.01),S1(r =-0.512,P<0.05),S3(r =-0.715,P<0.05),S4(r =-0.583,P<0.05),b/a(r =-0.470,P<0.05)have a negative correlation.In multiple linear regression with age as the dependent variable,the characteristic parameters K,T4(CT),and S3 were independent contributors to the prediction of arteriosclerosis.Conclusion:In this study,photoelectric pulse sensor is used to realize non-invasive detection of pulse wave for evaluation and predictive analysis of arteriosclerosis,which lays a certain foundation for the auxiliary diagnosis system for non-invasive detection of arteriosclerosis.The results of this study show that the pulse wave waveform can objectively reflect the degree of arteriosclerosis and hemodynamic changes,and change the pulse wave with the deterioration of blood vessel elasticity,blood viscosity and arteriosclerosis.The K,CT,PPT,S4 values can predict to a certain extent and reflect the degree of arteriosclerosis,which can be used as auxiliary indicators for diagnosing the degree of arteriosclerosis.The characteristic parameters K,CT and S3 were better than other characteristic parameters at different ages,which indicated that to a certain extent,they could predict and reflect the occurrence of arteriosclerosis with age,and could be used as auxiliary indicators to provide scientific reference for non-invasive screening of early arteriosclerosis. |
