Objective:The relationship between hyperglycemia and inflammation was investigated by collecting blood glucose,C-reactive protein to albumin ratio, neutrophil to lymphocyte ratio(NLR),neutrophil to platelet ratio(NPR),and platelet-lymphocyte ratio(PLR)of patients with obstructive and non-obstructive myocardial infarction upon admission. It provides a new idea to judge the clinical difference of patients. Methods:Clinical data of patients intended to be diagnosed with AMI in the First Affiliated Hospital of Xinjiang Medical University from January 1,2020 to December 31,2020,who received coronary angiography within 48 hours after admission and met the inclusion criteria, were selected for statistical analysis. Results: 1.Comparison of general data: the median age of patients in the MINOCA group was younger than that in the MI-CAD group, and the proportion of women was higher, and the difference was statistically significant(P <0.001).The proportions of diabetes mellitus and smoking history in MI-CAD group were higher than those in MINOCA group, and the differences were statistically significant(P< 0.05).2.In MI-CAD group, there were significant differences between hyperglycemia and WBC,NEU,LYM,Hba1 c,NLR,NPR,CRP/ALB ratio and LVDd(P < 0.05).3.Comparison of biochemical laboratory results between hyperglycemic MI-CAD group and MINOCA group: glycosylated hemoglobin in MI-CAD group showed significant difference(P < 0.05).There were significant differences in glycosylated hemoglobin and BNP in MINOCA group(P <0.05).4.Comparison of inflammatory indexes between hyperglycemia MI-CAD group and MINOCA group: there were significant differences in hyperglycemia and WBC,NEU,LYM,NLR,NPR,CRP/ALB ratio,LVDd in MI-CAD group(P < 0.05);There was no statistically significant difference in the indicators within the MINOCA group.5.Spearman linear correlation analysis between each index and blood glucose: In MI-CAD group,WBC,NEU,Hba1 c,CK-MB postoperative, troponin postoperative,IL-6,NLR,NPR,CRP/ALB ratio were positively correlated with hyperglycemia, and the correlation coefficients with hyperglycemia were 0.507,0.513,0.187,0.158,0.18,respectively.0.199,0.574,0.519,0.263,P < 0.05).Other indicators were not correlated with hyperglycemia. In the MINOCA group, hyperglycemia was positively correlated with Hba1 c,LDLC and D-dimer,and the correlation coefficient was(R=0.049,0.348,0.520,0.395,P < 0.05).There was no correlation between the other indicators(P > 0.05).Conclusion: 1.High blood glucose level is significantly correlated with WBC,NEU,LYM,NLR and NPR inflammatory indicators in patients with MI-CAD.2 High blood sugar in MI-CAD patients, whether with or without diabetes, suggests more serious inflammation and heart damage.3 MINOCA patients showed mild myocardial injury, and there was no significant correlation between admission blood glucose and inflammatory response.
