Objective: To explore the relationship between fibrinogen(FIB)and monocyte to lymphocyte ratio(MLR)and coronary plaque vulnerability in patients with coronary heart disease.Methods: Collected from January 2013 to July 2021 in the the department of Cardiology of the first affiliated hospital of Xinjiang Medical University diagnosed with coronary heart disease and performed intravascular ultrasound(IVUS)and fibrinogen,monocyte count,lymphocyte count and other patients,according to the inclusion and exclusion criteria and IVUS,118 cases of stable plaque group and 87 cases of unstable plaque group.Using the general data questionnaire,the area under the ROC curve was used to assess the diagnostic value of fibrinogen and MLR for unstable plaques,and logistic regression analysis was used to analyze the risk factors of unstable plaques.Results: The area under the curve for the fibrinogen combined with the MLR was 0.725(95% CI:0.656-0.794;P<0.001).Group the FIB-MLR 0: MLR 0.26 and FIB≤3.26g/L,n= 40;FIB-MLR 1: MLR> 0.26 or FIB>3.26g/L,n = 87;FIB-MLR 2: MLR> 0.26 and FIB>3.26g/L,n = 78.Among the three groups were no general clinical data,coronary angiography measurements and plaque composition(P<0.05)and lumen area and plaque burden(P<0.05).Logistic Multivariate regression analysis showed diabetes mellitus,lymphocyte count,MLR,fibrinogen,LDL,and FIB-MLR as independent risk factors for unstable plaques in patients with coronary heart disease(P<0.05).Conclusion: 1.fibrinogen and monocyte count increase the instability of vascular plaques in patients with coronary heart disease.2.Fibrinogen and MLR are useful for screening vulnerable plaques in coronary arteries.3.Combining fibrinogen and monocyte to lymphocyte ratio with IVUS examination is helpful to judge the vulnerable plaques of coronary arteries.
