Objective: The aim of this study was to compare the surgical related indexes of Da Vinci surgical system(DVSS)and laparoscopic surgery under different conditions and to analyze the advantages of DVSS in gynecological total hysterectomy.Methods: Patients who underwent total hysterectomy for benign gynecological diseases from July 2020 to July2022 at the People’s Hospital of the Autonomous Region were used as the study population,from which patients undergoing combined surgery for pelvic organ prolapse and intraoperative detection of malignant tumors were excluded.Three hundred and fifteen patients each were selected for da Vinci robotic hysterectomy(RH)and laparoscopic hysterectomy(LH).The surgical indices related to RH and LH were compared in patients with advanced age,obesity,uterus ≥12 weeks,intraoperative adhesions,and history of abdominal surgery,respectively.Results: In patients with advanced age,uterus ≥12 weeks,intraoperative adhesions,and history of abdominal surgery,it could be concluded that the RH group had less estimated intraoperative bleeding(EBL),shorter postoperative hospital stay(LOH),and postoperative drain removal time than the LH group,and all differences were statistically significant,however,in terms of operative time(OT),the RH group took longer than the LH group,and there was a statistical difference.In obese(BMI ≥ 28.0 kg/m2)patients,although intraoperative EBL,postoperative LOH and postoperative drain removal time were consistent with the results in other conditions,and all of them were statistically different.However,no significant differences were seen between the RH and LH groups in OT,and the differences were not statistically significant.The preoperative-postoperative hemoglobin difference and postoperative complication rates were not significantly different in each of the different conditions.Conclusions: The da Vinci robotic Xi surgical system is safer and more effective than laparoscopic surgery in patients with advanced age,obesity,uterus ≥12 weeks,intraoperative adhesions,and history of abdominal surgery.
