Objective: To investigate the correlation between cranial diffusion tensor(DTI)and cognitive impairment in primary normobaric hydrocephalus(INPH).Methods: From January 2022 to January 2023,20 patients with INPH who were hospitalized in the neurosurgery department of the First Affiliated Hospital of Xinjiang Medical University were selected.ROI regions were manually plotted on the lateral ventricle(radial crown),the knee of the corpus callosum, and the compression part of the corpus callosum, and superimposed on FA,ADC,and MD maps to obtain DTI values(i.e.,FA,ADC,and MD values),and MMSE values were obtained. The correlation between DTI values and MMSE values was analyzed using Pearson correlation. The ROC curve is then used to analyze whether DTI parameters can determine whether there are responders or non responders in the cerebrospinal fluid release experiment. Results: There was a positive correlation between FA values and MMSE scores in various interest intervals in patients with INPH.There is a correlation between the MD values of the genu and compression portions of the corpus callosum and the MMSE scores, and this correlation presents a negative correlation. There was no correlation between the MD value around the ventricles and the MMSE score(P>0.05).There is a correlation between the ADC value of the knee of the corpus callosum and the MMSE score, which presents a negative correlation. There was no correlation between the ADC values in the compression part of the corpus callosum and the periventricular area and the MMSE score(P>0.05).The diagnostic value of DTI quantized values(FA value,MD value,ADC value)in various regions of interest in patients with INPH in distinguishing between responders and non responders to cerebrospinal fluid release testing is not high(P>0.05).Conclusion: DTI is a reliable method for detecting hydrocephalus, and white matter damage in patients with INPH is the basis of cognitive impairment. |
