Clinical Characteristics And TCM Syndrome Patterns Of Renal Function Impairment In COVID-19 Patients

Objective:To explore the clinical characteristics and TCM syndrome patterns of renal function impairment in COVID-19 patients.Methods: We did a multi-center retrospective clinical study of adult patients with confirmed COVID-19,who had been hospitalized in Xinjiang Uygur Autonomous Regions COVID-19 designated hospital between July 2020-September 2020 and October2022-December 2022.All patients were collected the general data,clinical symptoms,TCM syndrome,laboratory indicators and prognosis indicators.The data are statistically analyzed using SPSS25.0 software and are considered statistically significant at P <0.05.Results: In total,498 hospitalized patients with COVID-19 were collected.During hospitalization,23.29%(116 / 498)of patients had renal impairment,and 61.21%(71 /116)were male.Male to female ratio of 1.58:1.3.82%(19 / 498)of the patients progressed to AKI.5.42%(27 / 498)of patients had elevated Scr index and 22.89%(114 /498)of patients had elevated BUN index.Proteinuria was present in 17.74%(82 / 498)of patients and hematuria was present in 21.17%(105 / 498)of the patients.COVID-19 patients with renal function impairment does not prolong the overall hospital stay.Male,advanced age,previous respiratory disease,ICU treatment are an independent risk factor for impaired kidney function and AKI in COVID-19 adult patients.In terms of TCM syndrome types,cold and humidity syndrome,damp and heat syndrome,qi and Yin two deficiency syndrome,and wind and heat syndrome are the main TCM syndrome types of this epidemic.On this basis,patients with renal impairment can progress into internal closed and external syndrome,wet impregnation,geomantic fighting and other syndrome types.Conclusion: COVID-19 combined with renal function impairment is still relatively high in clinical practice,which needs much attention.The identification of dangerous population as soon as possible,regular kidney function,kidney ultrasound and routine urine examination are important ways to protect the COVID-19 patients.The TCM syndrome type of COVID-19 patients with renal function impairment is mainly COVID-19 basic syndrome type.On this basis,it can be developed into internal closure and external syndrome,wet immersion syndrome,feng shui stroke and other syndrome.The TCM treatment of COVID-19 patients with renal function impairment should still focus on the treatment of COVID-19.
