| Objective:To observe the effects of Yu yin and Qian Yang method on blood pressure,blood pressure variability,TCM symptoms,Hcy level and sleep quality in patients with H-type hypertension(Yin deficiency and Yang hyperactivity type)with insomnia,to evaluate the clinical efficacy and safety of Yu yin and Qian Yang method treatment,and to provide more clinical information and therapeutic modalities for the treatment of H-type hypertension(Yin deficiency and Yang hyperactivity type)with insomnia.Methods:Sixty patients who attended the outpatient clinic of Cardiovascular I Department of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine between December 2021 and December 2022 and met the inclusion criteria were randomly divided into control and treatment groups according to the numerical table method,with 30 patients in each group.The control group was treated with conventional Western medicine,and the treatment group was treated with the self-prepared soup plus reduction based on the control group,and the treatment course was 4weeks.The blood pressure(office blood pressure,ambulatory blood pressure,blood pressure variability),TCM syndrome score and efficacy,plasma Hcy level and sleep quality score were recorded before and after treatment in both groups,and SPSS25.0 statistical analysis was applied to evaluate the effectiveness and safety of treatment.Results:1.General data(gender,age,disease duration,weight,etc.)of patients in both groups before treatment,the differences were not statistically significant(P>0.05)and could be compared.2.After treatment,the TCM symptom scores of both groups decreased,and the difference was statistically significant(P<0.05),while patients in the treatment group were better than the control group in terms of dizziness,headache,insomnia,back pain and total score,and the difference was statistically significant(P<0.05).3.All the indexes of office blood pressure,ambulatory blood pressure and blood pressure variability decreased in both groups compared with those before treatment,and the difference was statistically significant(P<0.01),among which the decrease in office blood pressure,24 h SBP,DSBP,DDBP,NSBP,24 h blood pressure standard deviation,daytime blood pressure standard deviation,nighttime systolic blood pressure standard deviation,and daytime and nighttime systolic blood pressure variability coefficient was better in the treatment group than in the control group(P<0.05);the efficiency of office blood pressure was 87% in the treatment group and 77%in the control group,and the treatment group was better than the control group(P<0.05).4.Plasma Hcy levels in both groups were reduced after treatment levels(P < 0.01),and the treatment group was better than the control group,and the difference was statistically significant(P<0.01).5.The single and total PSQI scores decreased in both groups after treatment(P<0.01,P<0.05),and the treatment group was better than the control group in several items of sleep quality,time to sleep,sleep efficiency,and total PSQI score(P<0.01,P<0.05),and the sleep efficacy efficiency was 73.3% in the control group and 93.3% in the treatment group,which was better than the control group(P<0.01).6.There was no change in safety indexes before and after treatment,and the difference was not statistically significant(P>0.05).Conclusions:1.Under the conventional western medicine treatment,the combination of yu yin and yang method can improve the TCM symptoms and sleep quality in patients with H-type hypertension(Yin deficiency and Yang hyperactivity type)with insomnia,and the overall efficacy is better than the western medicine treatment alone.2.The method of nurturing Yin and submerging Yang can reduce Hcy level and blood pressure in patients with H-type hypertension(Yin deficiency and Yang hyperactivity type)with insomnia,stabilize blood pressure fluctuations,and the overall efficacy is better than that of Western medicine treatment alone.3.No significant adverse effects were found throughout the study,so the method deserves to be studied in depth with a view to obtaining better treatment results. |
